先兆早产合并妊娠期糖尿病患者的药学监护

Pharmaceutical care on a patient with threatened premature labor accompanied with gestational diabetes mellitus

目的探讨临床药师为先兆早产合并妊娠期糖尿病患者提供的药学监护和工作切入点。方法临床药师参与1例先兆早产合并妊娠期糖尿病患者的治疗过程,结合患者妊娠期特点,拟定药学监护计划,评估治疗效果,优化药物治疗方案。结果与结论临床药师参与妊娠期患者的用药管理与个体化用药方案的制定,为患者提供全程化药学服务,协助临床处置药物不良反应,使患者治疗获益得到最大化。

Objective To explore how to provide pharmaceutical care for patients with threatened premature labor accompanied with gestational diabetes mellitus. Methods The clinical pharmacist participated in the treatment of a patient with threatened premature labor accompanied with gestational diabetes mellitus,made the plan of pharmaceutical care considering the characters in pregnancy,evaluated the efficacy of therapy and optimized the medical regimen. Results and Conclusion Pharmaceutical care provided by clinical pharmacists is quite valuable in guaranteeing the rationality and safety of medication. Clinical pharmacists participate in the management of pregnant patients＇ medication,help develop individualized dosage regimen,provide whole-ranged pharmaceutical care,assist in dealing with adverse drug reactions,and therefore maximizing patient＇ benefit.
