宝石能谱CT成像在不同组织来源、病理类型肿瘤及其转移淋巴结中的诊断价值 被引量:27

Study on the significance of precious stone energy spectrum CT imaging in diagnosis of different tissue sources and pathological types of tumors and metastasis in lymph nodes

摘要 目的针对宝石能谱CT成像在不同组织来源、病理类型肿瘤及其转移淋巴结中的诊断价值进行研究。方法随机选择2013年12月至2015年9月期间收治的食管癌、胃癌、肺癌患者60例,作为本次研究的对象,采用宝石能谱CT成像技术对不同肿瘤组织的原发病灶、淋巴结转移、以及转移淋巴结中的有效原子序数、Ct值、碘基含量等进行检测,并进行相关的统计比较。结果不同病理类型以及原病灶淋巴结转移(小细胞肺癌、肺腺癌、肺鳞癌、小细胞肺癌转移淋巴结、肺腺癌转移淋巴结、肺鳞癌转移淋巴结、胃癌-食管癌转移淋巴结)的CT能谱特征参数值存在着统计学差异(P<0.05)。小细胞肺癌的不同能量keV下的CT能谱特征参数值与肺腺癌、肺鳞癌的比较普遍较低,随着能谱能量的升高,CT能谱特征参数值逐渐降低,部分keV间差异有统计学意义(P<0.05)。不同病理类型、淋巴结转移等患者的淋巴结碘含量、CT能谱衰减曲线斜率的比较,小细胞肺癌最低,肺鳞癌转移淋巴结、胃癌-食管癌转移淋巴结较高,部分病理类型间比较存在着显著差异(P<0.05)。而不同病理组织和原病灶淋巴结转移组患者的有效原子序数值比较,无显著差异(P>0.05)。结论宝石能谱CT成像技术,在不同组织来源、病理类型肿瘤及其转移淋巴结的诊断中,有一定的临床诊断价值。 Objective To explore the significance of precious stone energy spectrum CT imaging in diagnosis of tumors from different tissue sources and pathological types and metastatic lymph nodes. Methods A total of 60 patients with esophageal cancer,lung cancer or gastric cancer in this hospital during December 2013 to September 2015 were randomly selected as study group,and precious stone energy spectrum CT imaging had been applied for examination of primary lesions in patients with different tumor tissues,and metastatic lymph nodes as well as the effective atomic sequence of metastatic lymph nodes,CT value and iodine content were tested and compared with related statistical analysis. Results In this study,the difference in CT values of characteristic spectrum parameters in different pathological types of carcinomas( small cell lung cancer,lung adenocarcinoma,lung squamous carcinoma,gastric cancer,esophageal cancer and their metastasis in lymph nodes) was statistically significant( P〈0. 05). The values of different energy ke V CT spectrum characteristic parameters of small cell lung cancer in comparison with those of lung adenocarcinoma and lung squamous carcinoma were generally lower,following the increase of value of energy spectrum,value of CT characteristic parameter was gradually reduced,and the difference between ke Vs was statistically significant( P〈0. 05). Patients with different pathological types and iodine contents in metastatic lymph nodes,in comparison with CT attenuation curve slope spectrum,it in metastasis of lung small cell cancer and squamous carcinoma in lymph nodes and metastasis of stomach-esophageal cancer in lymph nodes was higher,and the difference was significant in comparison with pathological types( P〈0. 05). The difference in effective numerical number of atomic sequence in comparison with different pathological tissues and lesions with metastasis in lymph nodes was not significant( P〈0. 05). Conclusion The application of precious stones energy spectrum CT imaging has certain significance in diagnosis of tumors with different tissue sources,pathological types and metastasis in lymph nodes.
