腹部B超与阴道B超在异位妊娠诊断中的比较分析

The Comparative Analysis of Abdominal Ultrasound and Vaginal Ultrasound in the Diagnosis of Ectopic Pregnancy

目的观察腹部B超与阴道B超在异位妊娠诊断中的临床价值。方法选择2010年1月至2015年6月于我院进行异位妊娠治疗的162例患者作为研究对象,患者来诊后分别给予腹部B超和阴道B超检查,以患者术后病理结果作为最终诊断标准,观察腹部B超和阴道B超对异位妊娠检查的准确性,以及对超声图像进行比较。结果 162例异位妊娠阴道B超和腹部B超检查结果与术中直视及术后病理结果进行比较,腹部B超138例符合诊断,诊断准确率为85.18%,阴道B超152例符合诊断,诊断准确率为93.83%。阴道B超的准确率明显高于腹部B超,且具有统计学差异(P<0.05)。阴道B超对附件包块、胚芽、心管搏动、盆腔积液与假孕囊的检出率均高于腹部B超,且具有统计学差异(P<0.05)。结论阴道B超对异位妊娠检查结果有更高的准确率,而且对孕囊内结构的检查更加清楚。

Objective To observe the clinical value of abdominal B ultrasound and vaginal B ultrasound in the diagnosis of ectopic pregnancy. Methods 162 cases with ectopic pregnancy, which admitted in our hospital during January 2010 - June 2015, were selected as the research object. They were given the abdominal B ultrasound and vaginal B ultrasound examination. In patients with postoperative pathologic results as the final diagnosis standard, abdominal B ultrasound and vaginal B ultrasound for ectopic pregnancy test accuracy were observed and ultrasonic image were compared. Results 162 cases of ectopic pregnancy vaginal B ultrasonic and abdominal B ultrasonic examination results were compared with intraoperative direct and postoperative pathologic results. 138 cases of abdominal B ultrasound conforms to the diagnosis, diagnostic accuracy was 85.18%, 152 cases of vaginal B ultrasound conforms to the diagnosis, diagnostic accuracy was 93.83%. Accuracy of vaginal B ultrasound was obviously higher than that of abdominal B ultrasound, the difference was significant statistically significant （P〈0.05）. Detection rate of vaginal B ultrasound for accessory bag piece, germ, heart beats tube, pelvic cavity accumulates fluid, and the false pregnancy sac were higher than abdominal B ultrasound, the difference was significant statistically significant （P〈0.05）. Conclusion Vaginal B ultrasound for ectopic pregnancy test results has higher accuracy and structure within the pregnant bursa check more clearly.
