某三甲医院近3年住院费用中药费比例调查分析

Investigation and Analysis on the Proportion of the Drug Expenses in Hospitalization Cost in a Provincial Hospital during the Past Three Years

目的：了解医院药品费用比例情况,为卫生主管部门及医院领导降低药费决策提供依据.方法：收集武汉市某三甲医院2013年1月～2015年12月3年共25725例住院病人的总医疗费用金额、药品费用金额数据.通过分析该院药品费用占医疗总费用的百分比,并对照国家政策规定的三甲医院药品费用百分比标准,来衡量该院药品费用比例是否恰当.结果：该医院药品费用比例偏高.“以药养医”的历史原因、药品的虚高定价、药品的不合理使用以及医务人员伦理道德水平的滑坡是造成药品费用比例偏高的主要因素.结论：呼吁政府加大对医院补偿机制的建立,降低药价,严打医药领域不正之风.

Objective:To understand the proportion of the expenses on medical drugs and provide reference for reducing drugs costs by health authorities' and hospital leaders' decision-making.Methods:Collect the data of total medical expenses and the expenses on medical drugs for all the inpatients(n=25725)in a provincial hospital during the past three years from January 2013 to December 2015.The proportion of the expenses on medical drugs is measured by the percentage of drugs expenses accounting for the total expenses of hospitalization.By comparing the proportion of the expenses on medical drugs in this hospital to that of the national standard,the appropriateness of the medicine expenditure in this hospital can be evaluated.Results:The proportion of the expenses on medical drugs in this hospital is higher than the upper limit of the national standard.The irrational use of drugs to support medical services,artificially high prices of drugs,inappropriate use of drugs,and the landslide of the medical workers' ethics level are the main causes of higher proportion of drug costs.Conclusion:We appeal to our government to increase the establishment of compensation mechanism in hospital and to reduce prices of medical drugs,as well as to crack down on unhealthy tendency in medical field.
