睾丸细针穿刺活检术对男性血清抗精子抗体的影响

Effect of fine needle aspiration biopsy of testis on serum anti-sperm antibody in male

目的探讨睾丸细针穿刺活检手术对男性血清抗精子抗体表达的影响。方法回顾性分析2012-01-2014-12间在门诊行睾丸细针穿刺活检手术的56例患者的临床资料,分析患者穿刺术前后的血清抗精子抗体的表达程度,比较穿刺术前后两者间的差异,评估睾丸细针穿刺活检手术对男性血清抗精子抗体的影响。结果 56例患者术后睾丸组织病理检测发现精子39例,未发现精子17例。手术前后的血清抗精子抗体总阳性率为7.14%(4/56)和8.93%(5/56),其中梗阻性无精子症者阳性率为7.69%(3/39)和10.26%(4/39),特发性无精子症者为5.88%(1/17)和5.88%(1/17)。手术前血清抗精子抗体Ig G(+)为3.57%(2/56),Ig M(+)为1.79%(1/56),Ig A(+)为1.79%(1/56),手术后血清抗精子抗体Ig G(+)为5.36%(3/56),Ig M(+)为1.79%(1/56),Ig A(+)为1.79%(1/56)。手术前后各指标比较差异无统计学意义(P>0.05)。结论睾丸细针穿刺活检手术前后男性血清抗精子抗体表达程度差异不大,它对男性血清抗精子抗体表达无明显影响。

Objective To investigate the effect of testicular fine needle aspiration biopsy on the expression of anti-sperm antibodies in the serum of men. Methods The clinical data of 56 patients undergoing testicular fine needle aspiration biopsy from January 2012 to December 2014 were retrospectively analyzed. The expression of serum anti-sperm antibodies of these patients was detected,and the difference of expression of serum anti-sperm antibodies was compared before and after the fine needle aspiration biopsy. Results The pathological examination of testicular tissues showed that 39 of 56 cases were found sperm after the puncture,and 17 cases were not found sperm. The total positive rate of anti-sperm antibodies was 7. 14%（ 4 /56） and 8. 93%（ 5 /56） before and after the puncture. The positive rate of anti-sperm antibodies was 7. 69%（ 3 /39） and 10. 26%（ 4 /39） for the obstructive azoospermia patients before and after the puncture,and 5. 88%（ 1 /17） and 5. 88%（ 1 /17） for idiopathic azoospermia patients. The positive rate in the serum was 3. 57%（ 2 /56） for Ig G,1. 79%（ 1 /56） for Ig M,1. 79%（ 1 /56） for Ig A before the puncture,and 5. 36%（ 3 /56）,1. 79%（ 1 /56）,1. 79%（ 1 /56） after the puncture,respectively. There was no significant difference in positive rate of anti-sperm antibodies before and after puncture（ P〈0. 05）. Conclusion Testicular fine needle aspiration biopsy has no significant effect on the expression of anti sperm antibodies in serum of male.
