针刺通里穴对脑血栓患者大脑前动脉、中动脉血流及PI、RI的影响

Acupuncture Tongli in patients with cerebral thrombosis anterior,middle cerebral artery blood flow,PI and RI

目的探讨针刺通里穴在不同时间点对大脑中动脉(MCA)和大脑前动脉(ACA)的Vs、Vd、Vm、PI、RI等血流动力学指标的影响。方法选取左侧偏瘫脑血栓患者30例。针刺左通里穴,使用TCD观察并记录进针前、进针时、行针时、出针时和出针后5个时间点的大脑前、中动脉Vs、Vd、Vm、PI及RI数据。结果 1)针刺通里穴时,病灶侧(右侧)MCA在行针时的Vd、Vm上升,具有统计学意义(P<0.05);病灶对侧(左侧)出针时的Vd、Vm明显上升,具有统计学意义(P<0.05);2)针刺通里穴,病灶侧(右侧)及对侧(左侧)的ACA各参数在各时间点均无统计学意义(P>0.05);3)针刺通里穴未引起双侧MCA和ACA的PI、RI变化(P>0.05)。结论针刺通里穴对动脉硬化性脑血栓患者的MCA具有正性调节及累积效应,对ACA无明显影响。

Objective Discuss acupuncture tongli at different time points to Vs,Vd,Vm,PI and RI of MCA and ACA the influence of hemodynamic parameters. Methods Select the left hemiplegia patients with ischemic cerebrovascular 30. Using TCD needle into the point,10 minutes before start time,acupuncture in patients with the left side of tongli acupoint,observe and record before entering the needle,when the needles are inserted,lines retaining needle,needle retaining needle at the time of the needle and the needle after,at other five time Vs,Vd and Vm,PI,RI and other hemodynamic parameters. Results 1） acupuncture tongli,lesion side（ right） MCA in needles of Vd,Vm,statistically significant（ P〈0. 05）; Lesions on the side（ left） Vd,Vm is obvious rise at the time off the needle,with statistical significance（ P〈0. 05）; 2） acupuncture in den,lesions（ right） and the contralateral side（ left） of the ACA Vs,Vd and Vm does not change significantly at all time points no statistical significance（ P〉0. 05）; 3） Tongli did not cause the anteromedial brain artery and artery PI and RI of change（ P〉0. 05）. Conclusion Acupuncture tongli of hardening of the arteries in patients with cerebral thrombosis and the regulation of the MCA has a positive cumulative effect,for no obvious accumulative or blood flow velocity of ACA.
