奥美拉唑致214例不良反应病例报告分析

Analysis of 214 Cases of Adverse Reactions Caused by Omeprazole

目的：分析奥美拉唑致不良反应的特点,供临床合理用药参考。方法：检索1991年1月1日-2014年12月31日大医医学搜索、万方医学网和中国医院数字图书馆有关奥美拉唑致不良反应的病例报告,进行统计与分析。结果：共检索到奥美拉唑致不良反应的病例报告214例,其中男性发生不良反应例数较多（155例,72.43%）;20~60（不含60）岁年龄段占比较高（165例,77.10%）;口服给药者（160例,74.77%）发生不良反应高于注射给药者（54例,25.23%）;不良反应累及器官或系统较为广泛,临床表现呈复杂化、多样化;主要的临床表现是全身反应、血液系统、消化系统和神经系统表现;严重的不良反应为过敏性休克和剥脱性皮炎。经停药（或对症处理）,214例患者均恢复正常,无死亡病例发生。结论：奥美拉唑所致不良反应的临床表现复杂多样,但以过敏反应比较多见,各科临床医生应给予更多关注。

Objective： To analyze the characteristics of adverse reactions caused by omeprazole and provide a reference for rational use of the drug in clinic. Methods ： The data on cases of adverse reactions caused by omeprazole reported from January 1st, 1991 to December 31 st, 2014 in Dayi Medical Search, Wanfang Med Online and CNKI,were collected for statistical analysis. Results ： A total of 214 case of adverse reactions caused by omeprazole were collected, of which 72.43%（155 cases） occurred in male patients, 77.10%（165 cases） in the patients at ages of 20 to 60（not including 60） years, 74.77%（160 cases） were caused by intravenous administration and 25.23%（54 cases） caused by oral medicine. The adverse reactions involved various organs and systemic, of which the clinical manifestations were complex and varied. The main clinical manifestations included systemic reactions as well as the manifestations of circulatory, digestive and neurologic systems. The serious adverse reactions included anaphylactic shock and exfoliative dermatitis. After withdrawal of the drug or after symptomatic treatment, all the 214 patients were recovered to normal, and no death occurred. Conclusion ： The clinical manifestations of adverse reactions caused by omeprazole is complicated and varied, most of which were allergic reactions. The manifestations should be paid more attention by clinicians.
