不同保温方法下膀胱冲洗液对经尿道前列腺电切术后膀胱痉挛的影响 被引量:4

摘要 目的探讨不同保温方法下膀胱冲洗液保温效果及对经尿道前列腺电切术(TURP)后膀胱痉挛的影响。方法105例前列腺增生行TURP的患者,随机分成对照组、观察组1、观察组2,各35例。对照组采用直接从恒温箱取出的等渗冲洗液;观察组1采用恒温箱取出的等渗冲洗液,外周套用特殊保温材料制成的保温套;观察组2采用在恒温箱取出的等渗冲洗液的两侧各贴有一片输液加热器,每3--4h更换一次。观察膀胱冲洗量、天数以及冲洗过程中膀胱冲洗液温度、膀胱痉挛的发生情况。结果观察组2保温效果优于对照组及观察组1,差异有统计学意义(P〈0.01);膀胱痉挛发生率观察组2低于观察组1,差异有统计学意义(P〈0.05),且低于对照组,差异有统计学意义(P〈0.01)。结论采取在恒温箱取出的等渗冲洗液的两侧各贴有一片输液加热器,每3~4h更换一次,能够维持膀胱冲洗液的温度相对恒定,减少患者膀胱痉挛的发生,提高患者的舒适度。 Objective To investigate the effects of washing fluid at different temperature for bladder washing on cystospasm after transurethral resection of the prostate (TURP). Methods A total of 105 prostatic hyperplasia patients underwent TURP were randomly divided into control group (n =35), observation group 1 ( n = 35) and observation group 2 ( n = 35). Washing fluid from thermotank was used for isotonic rinses in control group, while washing fluid with sheath made by insulation material and with infusion heating apparatus (replaced 3- 4 h/per time) were used in observation group 1 and 2, respectively. The flush amount, days of bladder washing, tem- perature of washing fluid and incidence of cystospasm were observed. Results Observation group 2 received better heat insulation effect and lower incidence of cystospasm compared with Observation group 1 and control group (P 〈 0.01), Conclusion washing fluid with infusion heating apparatus (replaced 3-4 h per time) is effective for maintaining the temperature of washing fluid, reducing the risk of cystospasm and improving comfortable degree in patients after TURP.
