摘要
目的观察三七总皂苷对重症急性胰腺炎(SAP)大鼠的治疗作用,并探讨其机制。方法选取SD大鼠30只,随机分为假手术组(SO组)、重症胰腺炎组(SAP组)、三七总皂苷干预组(PNS组),每组各10只,经胆胰管逆行注射5%牛磺胆酸钠制备大鼠SAP模型。SO组进腹后仅翻动胰腺和十二指肠后关腹,不注入5%牛磺胆酸钠,其中PNS组模型建立后立即予腹腔注射三七总皂苷50 mg/m L,SO组、SAP组建模后注射生理盐水0.1 m L/100g。各组大鼠分别于建模后4 h检测血清淀粉酶(AMS),光镜下观察胰腺组织病理学改变,免疫组化SP法检测胰腺组织中NF-κB及炎症因子TNF-α表达。结果与SO组相比,PNS组和SAP组大鼠血清AMS水平、胰腺组织病理学评分、NF-κB和TNF-α表达量显著升高(P均<0.05);与SAP组相比,PNS组大鼠血清AMS水平、胰腺组织病理学评分、NF-κB和TNF-α表达量显著降低(P均<0.05);相关性分析结果显示,胰腺组织腺泡细胞内NF-κB与TNF-α因子表达呈正相关(r分别为0.958、0.992、0.989,P均<0.05)。结论三七总皂苷能减轻SAP的胰腺损伤,其机制可能为抑制胰腺组织NF-κB因子激活,减少炎性细胞因子过度释放。
Objective To investigate the effect of panax notoginseng saponins in treatment of rats with severe acute pancreatitis( SAP),and to explore the mechanism. Methods Thirty SD rats were randomly divided into three groups:sham operation group( SO group),severe acute pancreatitis group( SAP group),PNS preconditioning group( PNS group),and there were 10 rats in each group. Rat SAP models were induced by retrograde injection of 5% taurocholic acid sodium to pancreatic duct. SO group was just treated with pancreas and duodenum turning after abdominal closure without injection of 5% taurocholic acid sodium. The rats in the PNS group were given intraperitoneal injection of 0. 1 m L /100 g PNS 50 mg / m L( xuesetong injection 0. 1 m L /100 g) after the models were established. And rats in the other two groups were given normal saline( 0. 1 m L /100 g). The serum AMS were detected at 4 hours after modeling,we observed the pathological changes of pancreatic tissues under the light microscopy. The expression of nuclear transcription factor-κB( NF-κB) and tumor necrosis factor-α( TNF-α) was detected by immunohistochemical SP method. Results Compared with the group SO,serum AMS levels,pathology scores of pancreatic tissues and the expression of NF-κB and TNF-α were significantly increased in the groups PNS and SAP( all P〈0. 05),and those in the SAP group rats were significantly higher than those in the PNS group( all P〈0. 05). The expression of NF-κB was positively correlated with TNF-α expression in the pancreas acinar cells( r = 0. 958,0. 992 and 0. 989,all P〈0. 05). Conclusions Panax notoginseng saponins may reduce the pancreatic injuries of SAP rats. Panax notoginseng saponins may have protective effect on SAP rats by inhibiting the activation of NF-κB and reducing the excessive release of inflammatory factors.
