沙美特罗替卡松粉吸入剂联合布地奈德治疗支气管哮喘急性发作期疗效观察

Clinical observation of the efficacy of seretide and budesonide to treat patients with acute bronchial asthma

目的：观察沙美特罗替卡松粉吸入剂联合布地奈德治疗支气管哮喘急性发作期的临床效果。方法选择支气管哮喘急性发作期患者180例，随机分为联合治疗组、沙美特罗替卡松粉吸入剂组、布地奈德组各60例。联合治疗组在常规治疗基础上给予沙美特罗替卡松粉吸入剂联合布地奈德雾化剂吸入治疗，布地奈德组给予布地奈德雾化剂吸入治疗，沙美特罗替卡松粉吸入剂组给予沙美特罗替卡松粉吸入剂吸入治疗，疗程均为7天。用药结束后比较3组患者咳嗽、呼吸困难、肺部啰音和喘息的消失时间、肺功能情况和日间、夜间症状评分。结果联合治疗组咳嗽、呼吸困难、肺部哮鸣音消失时间均较沙美特罗替卡松粉吸入剂组和布地奈德组明显缩短（P＜0．05）。联合治疗组治疗后肺功能指标第1秒用力呼气容积（FEV1）、用力呼气肺活量（FVC）、FEV1／FVC比率、最大呼气峰流速（PEF）改善程度优于沙美特罗替卡松粉吸入剂组和布地奈德组（P＜0．05）。联合治疗组治疗后日间及夜间症状评分改善程度优于沙美特罗替卡松粉吸入剂组和布地奈德组（P＜0．05）。沙美特罗替卡松粉吸入剂组上述各指标均优于布地奈德组（P＜0．05）。结论沙美特罗替卡松粉吸入剂联合布地奈德治疗支气管哮喘急性发作期较单一用药起效快、疗效更好，并可改善肺功能，值得临床推广应用。

Objective To assess the efficacy of seretide （ salmeterol xinafoate and fluticasone propionate powder for inhalation） and budesonide on patients with acute bronchial asthma .Methods A total of 180 patients with acute bron-chial asthma were randomly divided into a combined treatment group , a seretide group and a budesonide group （n=60）. The budesonide group was treated with budesonide , the seretide group was administrated with seretide , while the com-bined treatment group was given both .All patients were treated for seven days .Then, they were compared for the disap-pearance time of cough, dyspnea, rale and breathing, lung function, and the symptoms at daytime and night.Results Compared with the monotherapy groups , the disappearance time of cough , dyspnea and rale was remarkably reduced in the combined treatment group （P〈0.05）.The combined treatment group produced lung function indices such as forced expiratory volume in one second （FEV1）, forced vital capacity （FVC）, the ratio of FEV1/FVC, and peak expiratory flow （ PEF） as well as the scores of symptoms at daytime and night which were markedly improved compared with those in the monotherapy groups （P〈0.05）.The seretide group presented better indices shown above than the budesonide group （P〈0.05）.Conclusion The combination of seretide and budesonide can be adopted to treat acute bronchial asthma , with a faster onset , better efficacy and improved lung function in comparison with the monotherapy groups , which is wor-thy of clinical application .
