关节镜下空心螺钉固定治疗急性后交叉韧带胫骨止点撕脱骨折18例

Clinical observation on arthroscopic treatment of acute tibial avulsion fracture of posterior cruciate ligament with cannulated screw fixation technique under arthroscope

目的观察于关节镜下行空心螺钉固定治疗急性后交叉韧带胫骨止点撕脱性骨折的临床疗效。方法 18例急性后交叉韧带胫骨止点撕脱性骨折患者,于关节镜下复位,并经后内侧入路置入单枚带垫片直径4.0-4.5 mm空心钛质螺钉固定。结果 18例骨折均愈合,平均手术时间为（70.4±10.8）min,愈合时间（5.8±3.4）个月。术后1、3、6个月,本组患者的平均HSS分值分别为（44.3±5.6）、（83.7±5.4）和（93.6±6.2）分。结论关节镜下空心螺钉治疗后交叉韧带胫骨止点撕脱性骨折疗效确切。

Objective To observe the clinical effect of arthroscopic treatment of acute tibial avulsion fracture of the posterior cruciate ligament with screw fixation technique under arthroscopy. Methods 18 cases with acute tibial avulsion fracture of the posterior cruciate ligament underwent reduction under arthroscope and was inserted with hollow titanium screws with single gasket and with a diameter of 4.0-4.5 mm via posterior medial approach for fixation. Results All the patients were healed. The average operation time was（70.4±10.8） min, and average healing time was（5.8±3.4） months. 1, 3, 6 months after operation, the average HSS score of these patients were（44.3±5.6）,（83.7±5.4） and（93.6±6.2）, respectively. Conclusion The technique using cannulated screw to treat tibial avulsion fracture of posterior cruciate ligament under arthroscope is safe with satisfactory effect.
