应用线性探针方法快速检测结核分枝杆菌耐药性的效果评价 被引量:3

摘要 目的评估线性探针方法快速检测结核分枝杆菌的耐药性效果。方法选取2013年-2014年江西省耐药监测点分离培养的阳性菌株,收集菌体,提取DNA,采用MTBDRplus试剂盒进行检测,以国际标准的比例法检测结果作为对照,评价其敏感度及特异度,并分析突变分布情况。结果检测利福平耐药的灵敏度为93.33%,特异度为98.35%;检测异烟肼的灵敏度为76.31%,特异度为98.83%。耐利福平基因突变频率最高的位点为rpo B H531L(57.89%),耐异烟肼基因突变频率最高的位点是kat G S315T1(60.61%)。结论可利用MTBDRplus快速检测利福平和异烟肼的耐药性,是快速发现耐药结核分枝杆菌的有效方法。 Objective To evaluate the effect of MTBDRplus assay in rapid detection of resistance in Mycobacterium tuberculosis.Methods The positive strains,cultivated and isolated from the resistance monitoring spots in Jiangxi during 2013- 2014,were collected and DNA was extracted,which were detected by MTBDRplus test kit,with international standards of proportion method detection results as the control,to evaluate the sensitivity and specificity,and to analyze the mutation distribution. Results The sensitivity and specificity of MTBDRplus assay for the detection of RIF and INH resistance were 93. 33%,76. 31% and 98. 35%,98. 83% respectively. The H531 L mutation of rpoB was the most frequent( 57. 89%),the frequent mutations of katG gene promoter region was S315T1( 60. 61%) in INH- resistant isolates. Conclusion The MTBDRplus assay can be used for rapid detection of rifampin and isoniazid resistance,which is a fast effective method to find resistant Mycobacterium tuberculosis.
