全身磁共振弥散加权成像检测食管癌全身转移的可行性及临床价值

The Availability and Value of Whole-Body Diffusion-Weighted MRI in the Diagnosis of Systemic Metastasis in Esophageal Carcinoma

摘要 目的:探讨全身磁共振弥散加权成像在食管癌全身转移方面的诊断价值。方法48例食管癌患者行全身磁共振弥散成像检查,处理生成全身弥散图,并与相应患者全身SPECT结果相对照。结果48例经病理证实的食管癌患者中,全身弥散检出病灶170处,其中锁骨上淋巴结89处,纵隔淋巴结43处,腋下淋巴结22处,腹股沟淋巴结16处。肝脏转移者5例,肺转移者3例,脑、脊髓等其他部位转移者8例。SPECT发现转移淋巴结185处,其中锁骨上淋巴结93处,纵隔淋巴结51处,腋下淋巴结23处,腹股沟淋巴结18处。肝脏转移者5例,肺转移者5例,脑、脊髓转移者5例。结论全身磁共振弥散加权成像对恶性肿瘤的全身转移是一种有效的检查方法。 Objective To investigate the value of whole-body diffusion-weighted MRI in the diagnosis of systemic metastasis in esophageal carcinoma.Methods48 cases of esophageal carcinoma underwent whole- body diffusion-weighted MRI, and then the whole-body images were generated and compared with systemic single-photon emission computed tomography (SPECT).Results In 48 cases of pathologically confirmed esophageal carcinoma, 170 metastasis lesions were detected, including 89 lesions in supraclavicular lymph nodes, 43 lesions in mediastinal lymph nodes, 22 lesions in axillary lymph nodes, 16 lesions in inguinal lymph nodes, 5 lesions in liver, 3 lesions in lung and 8 lesions in other organs such as brain and bone marrow. SPECT showed 185 metastasis lesions, including 93 lesions in supraclavicular lymph nodes, 51 lesions in mediastinal lymph nodes, 23 lesions in axillary lymph nodes, 18 lesions in inguinal lymph nodes, 5 lesions in liver, 5 lesions in lung, 5 lesions in brain and bone marrow.Conclusion Whole-body diffusion-weighted MRI is an effective screening method for systemic metastases of malignant tumors.
