单核细胞CD40、血小板CD154及血清MCP-1在原发性高血压中的意义 被引量:2

摘要 目的观察单核细胞CD40、血小板CD154及血清MCP-1表达对原发性高血压患者病情和心脑血管并发症评估的意义。方法测定95例原发性高血压病患者和50例健康者单核细胞CD40、血小板CD154表达水平及血清单核细胞趋化蛋白-1(MCP-1)浓度。结果 EH组CD40和CD154表达水平、MCP-1浓度均显著高于对照组(P<0.001);MCP-1浓度与CD40和CD154表达水平均呈高度正相关(R2=0.4738、0.7375;均P<0.001)。高血压II级组CD40和CD154表达水平、MCP-1浓度均显著高于Ⅰ级组,Ⅲ级组显著高于Ⅱ级组(P<0.01)。EH并发心血管病组和并发脑血管病组CD40和CD154表达率、MCP-1浓度均显著高于单纯高血压组(P<0.001);三者在两并发症组之间无显著差异(P>0.05)。结论单核细胞CD40、血小板CD154及血清MCP-1水平可作为原发性高血压进展和心脑血管并发症评估的重要指标。 Objective To examine the clinical correlation of the levels of monocyte CD40,platelet CD154 and serum monocyte chemoattractant protein-1( MCP-1) expression to disease progression and cardiovascular and cerebrovascular complications in patients with essential hypertension. Method The levels of monocyte CD40 and platelet CD154 expression and serum MCP-1 concentration were determined in 95 EH patients and 50 healthy volunteers. Findings In group EH,the levels of CD40,CD154 and MCP-1 were significantly higher than those of the control group( P〈0. 001). The MCP-1 level was found to have significant positive correlation with CD40 and CD154 expressions( R^2= 0. 5922 and 0. 7977; P〈0. 001). In group EH at grade II,the levels of CD40,CD154 and MCP-1 were markedly higher than those of group at grade I,but remarkably lower than those of the group at grade III( P〈0. 01). The levels of CD40,CD154 and MCP-1 in the group with cardiovascular complications and the group with cerebrovascular diseases were both notably higher than those of the group with single hypertension( P〈0. 01). No statistically significant difference was found between the two complication groups( P〉0. 05). Conclusion Monocyte CD40,platelet CD154 and serum MCP-1 can serve as efficient indicators for evaluating disease progression and cardiovascular and cerebrovascular complications in essential hypertension patients.
