B超定位在新生儿脐静脉置管术中的应用效果研究

目的探讨B超定位在新生儿脐静脉置管术中的临床应用价值。方法选取我院2015年1～8月使用B超定位指引下进行脐静脉置管术的新生儿60例为观察组,另选取本院2014年1～12月行常规脐静脉置管术的新生儿90例为对照组,进行回顾性分析,比较两组置管情况。结果观察组60例中,置管成功52例（86.7%）,失败8例（13.3%）;52例置管成功新生儿中,43例置管于T_（8～9）,3例置管于T_7,6例置管于T_（10）。对照组90例中,置管成功63例（70.0%）,失败27例（30.0%）;63例置管成功新生儿中,31例置管于T_（8～9）,9例置管于T_7,16例置管于T_（10）,7例置管于T_（11）;两组置管成功率差异具有统计学意义（P〈0.05）。新生儿置管失败因素与脐静脉壁纤薄及早产密切相关。结论 B超定位在新生儿脐静脉置管术中具有较好的临床应用价值,能有效提高置管精确度,降低新生儿X线损伤,值得临床推广应用。

Objective To investigate the clinical application value of B ultrasound in neonatal umbilical vein cauterization.Methods Sixty cases of umbilical vein cauterization from August 2015 to January 2015 were selected as the observation group,and90 cases of normal umbilical vein cauterization were selected as the control group.The two groups were retrospectively analyzed.Results 52 cases（86.7%）among 60 in the observation group had successful cauterizations and 8 cases（13.3%）failed.Among 52 successful cases,there were 43 cases of T_（8～9）,3 cases of tube in T_7 and 6 case of T_（10）.In the control group,there were 63（70.0%）successful cases and 27（30.0%）failure cases.Among the successful cases,there were 31 cases of T_（8～9）,9 cases of T_7,16 cases of T_（10）and 7 cases of T_（11）.The difference in successful rates between the two groups had statistically significance（P 〈0.05）.Conclusion The application of B ultrasound in neonatal umbilical vein catheterization has good clinical value,can effectively improve the accuracy of the tube and effectively reduce the neonatal X-ray damage,therefore it is can be used widely in clinical application.
