参芪四虫汤联合当归补血汤加减治疗椎基底动脉供血不足性眩晕的临床效果

Clinical effect of Shenqi Sichong Decoction combined with Danggui Buxue Decoction in the treatment of vertebrobasilar insufficiency vertigo

目的探讨参芪四虫汤联合当归补血汤加减治疗椎基底动脉供血不足性眩晕的临床效果。方法选择辽宁中医药大学附属第二医院2014年2月~2015年2月收治的椎基底动脉供血不足性眩晕患者90例,按照随机数字表法分为两组,各45例。治疗组采用中医参芪四虫汤联合当归补血汤的治疗方案,对照组给予口服尼莫地平片配合丹参注射液静脉滴注治疗。观察两组治疗效果、治疗前后椎动脉和基底动脉收缩期峰流速（Vs）、舒张期末流速（Vd）、平均流速（Vm）的变化及血液流变学指标。结果治疗组总有效率为95.6%,对照组总有效率为82.2%,差异有统计学意义（P〈0.05）。治疗组治疗后椎动脉和基底动脉Vs、Vd、Vm均较治疗前及对照组显著提高,差异有统计学意义（P〈0.05）;对照组治疗后椎动脉Vs较治疗前提高,差异有统计学意义（P〈0.05）。治疗组治疗后全血比黏度、血浆比黏度、血细胞比容、红细胞电泳均较治疗前及对照组改善,差异有统计学意义（P〈0.05）。结论参芪四虫汤联合当归补血汤可有效改善椎基底动脉供血不足性眩晕患者的血管通透性和脑部微循环,治疗效果显著。

Objective To investigate clinical effect of Shenqi Sichong Decoction combined with Danggui Buxue Decoction in the treatment of vertebrobasilar insufficiency vertigo. Methods 90 patients with vertebrobasilar insufficiency vertigo in the Second Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from February 2014 to February 2015 were selected and divided into two groups according to the random number table method, 45 cases in each group. Traditional Chinese medicine Shenqi Sichong Decoction combined with Danggui Buxue Decoction were used in treatment group, Nimodipine Tablets combined with Danshen Injection intravenous drip were applied in control group. Treatment effect, peak systolic velocity（Vs）, end diastolic velocity（Vd）, mean velocity（Vm） changes of vertebral artery and basilar artery, hemorheology indexes before and after treatment between two groups were observed. Results Total effective rate in treatment group was 95.6%, total effective rate in control group was 82.2%, with statistical difference（P〈 0.05）. After treatment, Vs, Vd, Vm of vertebral artery and basilar artery in treatment group were significantly higher than those before treatment and control group, with statistical differences（P〈 0.05）. After treatment, Vs of vertebral artery and basilar artery in control group was higher than that before treatment, with statistical difference（P 〈0.05）. After treatment, whole blood viscosity, plasma viscosity, hematocrit, erythrocyte electrophoresis in treatment group were better than those before treatment and control group, with statistical differences（P 〈0.05）. Conclusion Shenqi Sichong Decoction combined with Danggui Buxue Decoction can effectively improve vascular permeability and brain microcirculation of patients with vertebrobasilar insufficiency vertigo, treatment effect is significant.
