门诊西药房调剂差错原因分析及防范对策

Analysis of the Causes of Dispensing Errors in Outpatient Pharmacy and Preventive Measures

目的分析门诊西药房调剂出现差错的原因,并提出防范对策。方法分析我院2013年1月~2014年1月处方资料,统计出现的调剂差错,并分析原因,制定相应的防范措施,对比2014年1月~2015年1月处方资料差错情况。结果制定相应的防范措施之后,在2014年1月~2015年1月门诊西药房调剂差错发生率为8.27%,低于2013年1月~2014年1月的53.33%,数据差异具有统计学意义（P〈0.05）。结论全面分析调剂差错,并提出相应的防范措施,可以降低调剂差错的发生率。

Objective To analyze the reasons of outpatient pharmacy dispensing errors, and puts forward the preventive countermeasures. Methods Analysis of prescription data in our hospital from January 2013 to January 2014, dispensing error statistics, and analyze the causes and formulate corresponding preventive measures, prescription data errors compared to January 2014 to January 2015. Results Formulate corresponding preventive measures, from January 2014 to 2015 years outpatient pharmacy dispensing error occurrence rate of 8.27%, lower than in January 2013 to 2014 January 53.33%, the data difference has statistical significance（P〈0.05）. Conclusion A comprehensive analysis of dispensing errors, and puts forward the corresponding preventive measures, can reduce the incidence of dispensing errors.
