摘要
肌力(muscle strength)即肌肉收缩力量,是反映人体运动功能的一个重要指标。肌力评定具有十分重要的临床意义,目前常用的肌力检查法有徒手肌力测定法、等张收缩测定法及等长收缩测定法,但这些测定方法均有不足之处。等速肌力测定是在肢体被动地进行等速运动时,通过测定反映肌肉负荷的系列参数,来评定肌肉的功能状态。该法既客观准确,又简便易行,且安全可靠。人体本身无法产生等速运动,须将肢体固定于仪器的杠杆,当其自主运动时,仪器的限速装置则会根据肢体力量的大小,随时调节杠杆对肢体的阻力,以保持肢体的运动速度维持在一恒定值。因此,肢体力量越大,杠杆的阻力就越大,则肌肉的负荷越强;反之亦然。此时,如测定反映肌肉负荷的系列参数,则可评定肌肉的功能状态。70年代出现的等速肌力测试训练仪就是根据上述原理设计的,目前,新型的等速肌力测试训练仪功能更加丰富,形成了一个全面的肌肉功能测试和训练系统。它由电脑、机械限速装置、打印机、座椅及附件组成;可测试力矩、最佳用力角度、肌肉做功量等多种参数,能全面反映肌力、肌肉爆发力、耐力以及关节活动度、灵活性、稳定性等多方面的情况。此方法准确可靠,并能提供等速向心、离心、被动等各种运动模式,是一种高效的运动功能评定和训练装置。可广泛适用于神经内科、神经外科、骨科、运动医学科及康复医学科等。我国80年代中期开始引进等速肌力测试训练仪,目前某些大医院和康复机构,以及体委已配置。最多应用的仪器型号有 Cybex、BIODEX、KIN-COM 及 LIDO,经营公司为香港科体健康及运动器材有限公司和香港启康国际医学仪器有限公司。
Muscle strength is an important index representing the motion functions of the human body.A muscle strength test has very important value in medicine.At present the methods for conducting a muscle strength test include:a manual muscle test,an isometric contraction test,and an isotonic contraction test.But these methods are not enough for the quantification of muscle strength.Recently a new method of testing muscle strength was performed by isokinetic exercise of the extremity.One can get a series parameters with the test and the function of muscle can be evaluated using these parameters.This method is safe,accurate,convenient,and quantifiable. The human body cannot do isokinetic exercise.Isokinetic exercise must be coerced using an isoki- netic dynamometer.The process involves the limb being fixed on a dynamometer lever with the limb and dy- namometer then moving together.The motion speed is arranged by the dynamometer and the resistance of the limb is regulated by it.The greater the strength of the muscle,the larger the resistance of the dynameter. Meanwhile the machine shows a series of parameters and represents the muscle funetion using the parameters. Isokineitcs is defined as the dynamic muscular contraction where the velocity of movement is controlled and maintained constant by a special device called an isokinetic dynamometer.The Isokinetic dynamometer consists of four parts:a computer,a velocity-limiting part,a printer,a chair and an appendix.The hip,knee, ankle,shoulder,elbow,wrist and back can be tested.The following modes are available:isokinetic concentric, isokinetic eccentric,isometruc,isotonic,and continuous passlve motion(CPM)modes.Information concerning a muscle’s strength,explosive force,endurance,range of motion is acquired.The isokinetic dynamometer is adaptable for Neurosurgery,Neurology,Orthopedic surgery,Sports medicine and Rehabilitation medicine. Isokinetic dynamometers have been used in China for more than ten years.Some large hospital such as CYBEX,BIODEX,KIN-COM,and LIDO have it.
