SPECT/CT对骨显像髋关节病变的诊断价值 被引量:4

摘要 目的探讨SPECT/CT对骨显像髋关节病变的诊断价值。方法 2013年10月至2014年10月进行99mTc-亚甲基二膦酸盐(MDP)全身骨显像的患者中,48例髋关节区异常放射性分布不能明确诊断,行SPECT/CT显像,并通过随访得到患者的病理诊断或综合影像诊断。分析SPECT、CT、SPECT/CT对患者病灶良恶性的诊断效能。结果 48例患者中,良性病灶34例,恶性病灶14例。SPECT、CT及SPECT/CT对48例患者的诊断准确性分别为70.7%、81.4%、95.6%,SPECT/CT分别与SPECT、CT比较差异均有统计学意义(P<0.05)。结论在对肿瘤患者进行骨显像,发现髋关节区病变以良性居多,SPECT/CT可提高诊断准确性。 Objective To evaluate the value of SPECT/CT in assessment of hip Joint lesions on bone scintigraphy. Methods Within patients who were receiving systemic 99 mT c-methylene diphosphonate( MDP) bone scintigraphy from October 2013 to October 2014,48 cases of abnormal radioactivity distribution in hip joint area can not be definitely diagnosed. SPECT / CT scintigraphy was carried and pathological diagnosis or comprehensive imaging diagnosis of the patient were obtained by follow-up. Diagnostic performance of SPECT、CT SPECT / CT in benign and malignant lesions was analyzed. Results 48 cases included benign lesions in 34 and malignant lesions in 14. The diagnostic accuracy by bone SPECT,bone CT and SPECT / CT in 48 patients were respectively 70. 7%,81. 4%,95. 6%. SPECT / CT respectively compared with SPECT,CT,differences were statistically significant( P〈0. 05). Conclusion Most of hip joint lesions were caused by benign diseases. SPECT / CT using 99Tcm-MDP has additional value than with SPECT or CT alone in differentiating malignancy from benignancy in hip joint lesion.
