摘要 目的研究MRI对脑萎缩认知正常(NL)个体发生阿尔兹海默症的风险价值。方法采用对照研究的方式对2011年12月~2013年12月神经内科收治的82例认知正常(NL)个体展开研究,所有个体均接受行纵向脑MRI成像检查,其中脑萎缩NL 42例作为研究组,NL40例作为对照组,对两组MRI检查结果进行分析比较。结果脑萎缩NL组阿尔兹海默症发生率为23.8%,NL组仅为5.0%,发生率比较X^2=14.48,P<0.05,具有显著差异性。右半球眶额皮质、左半球的内嗅皮层和后扣带回皮层发生率以及PCC的线性趋势低于左半球,P均<0.05,组间差异最为明显。结论脑萎缩NL患者发生阿尔兹海默症风险性高于NL患者,同时右大脑半球发生风险低于左大脑半球,OFC、EC以及PCC最明显。 Objective To study the value of the application of MRI in assessing the risk of Alzheimer's disease in individuals with encephalatrophy and normal learning( NL) ability. Methods 82 NL individuals treated in the Department of Neurology from December 2011 to December,2013,was studied by control study method,and all the individuals accepted vertical brain MRI examination,among whom 42 individuals with encephalatrophy and normal learning( NL) ability were treated as the study group,40 individuals with normal learning ability were treated as the control group,and the MRI results of the 2 groups were analyzed and compared. Results The occurrence rate of Alzheimer's disease in the encephalatrophy NL group was 23. 8%,while,the occurrence rate in the NL group was only 5. 0%,comparing the occurrence rate,X^2= 14. 48( P〈0. 05),indicating that there's significant difference. The occurrence rate of orbitofrontal cortex in the right hemisphere,entorhinal cortex and posterior cingulate cortex in the left hemisphere and the linear trend of PCC was lower than that in the left hemisphere,all the P values were less than 0. 05,indicating that the difference between 2 groups was the most significant. Conclusion The risk of Alzheimer's disease in the encephalatrophy NL group is higher than that in the NL group,and the risk in the right hemisphere is lower than that in the left hemisphere,especially,for OFC,EC and PCC.
