摘要 目的探讨使用Focus超声刀辅助解剖面神经主干在腮腺肿瘤手术的临床应用价值。方法需行腮腺手术患者69例,随机分为Focus超声刀组和电刀组,术中面神经的解剖均采用从主干到末梢的顺行法,行肿瘤及腮腺部分切除术,记录两组手术时间、术中出血量、术后48 h引流量、引流时间及术后并发症。结果 Focus超声刀组手术时间、术中出血量、术后48 h引流量、引流时间均低于电刀组,两组术后并发症差异无统计学意义。结论 Focus超声刀在腮腺手术中的应用是安全、有效、可行的,解剖面神经主干法更易显露和保护面神经,避免术后并发症。 Objective To investigate the clinical value of Focus harmonic scalpel assisted dissection of the facial nerve trunk in parotid tumor surgery. Methods Sixty-nine patients with parotid tumors were randomly divided into Focus harmonic scalpel group and electrocautery group. Facial nerve was dissected form the trunk to the tip during operation, and parotid tumors were resected. Operation time, amount of bleeding, volume of drainage within 48 hours after surgery, drainage time and postoperative complications were recorded. Results The use of Focus harmonic scalpel shortened the operation time and drainage time, reduced the amount of bleeding and volume of drainage compared with the electrocautery group. But there were no differences in postoperative complications between both groups. Conclusions The application of Focus harmonic scalpel in parotidectomy is safe, efficient and feasible. Dissection of the facial nerve trunk has the advantages of good exposure, protection of the nerve and prevention of complications.
