基于Quadrant-sextant通道工作系统的微创手术与传统开放手术治疗腰椎滑脱症的Meta分析

Minimally invasive surgery based on Quadrant-sextant working channel versus traditional open surgery for lumbar spondylolisthesis patients:a meta analysis

[目的]对基于Quadrant-sextant通道工作系统的微创手术与传统开放手术治疗腰椎滑脱症的疗效进行系统评价。[方法]检索MEDLINE、Pubmed、EMBASE、Web of science、中国生物医学文献数据库、中国知网全文数据库、万方数据库和维普数据库,收集经Quadrant-sextant通道工作系统微创手术和传统开放手术治疗腰椎滑脱的临床实验研究和观察性研究,运用Cochrane协作网及其Rev Man 5.2软件进行分析和评价。[结果与结论]经筛选,最后取符合纳入标准的文献12篇进行Meta分析,包括1 242例腰椎滑脱患者。Meta分析结果显示基于Quadrant-sextant通道工作系统微创手术治疗腰椎滑脱,在手术时间、术中出血量、术后住院时间、VAS评分、ODI指数及并发症上皆优于传统开放手术,提示基于Quadrant-sextant通道工作系统微创手术是一种较为理想的治疗腰椎滑脱的手术方式。

[Objective] To systematically review and compare the minimally invasive surgery based on the Quadrant- sextant working channel and traditional open surgery for the treatment of lumbar spondylolisthesis. [Methods] All the clinical experimental studies and observational studies from year 2000 to 2015 on the minimally invasive surgery based on the Quadrant-sextant working channel and traditional open surgery for the treatment of lumbar spondylolisthesis were retrieved and collected. Database we searched for this study included the MEDLINE,Pubmed,EMBASE,Web of science,Chinese Biomedical Database,China National Knowledge Infrastructure,Wan- Fang Database and Vip Database. These studies were analyzed and evaluated by Cochrane- net and Rev Man 5. 2 softeware. [Results] After screening out,total 12 studies met the inclusive criteria,involving 1242 cases of lumbar spondylolisthesis. The results of Meta analysis demonstrated that,based on the Quadrant-sextant working channel,the minimally invasive surgery for lumbar spondylolisthesis was superior to traditional open surgery in the operation time,intraoperative blood loss,postoperative hospitalization,VAS score,ODI score and side effects. [Conclusion] The minimally invasive surgery based on the Quadrant- sextant working channel is a comparatively ideal surgical procedure for lumbar spondylolisthesis.
