白内障超声乳化术后黄斑中心凹下脉络膜厚度的改变

Changes in subfoveal choroidal thickness after phacoemulsification

目的观察白内障超声乳化术后黄斑中心凹下脉络膜厚度(SFCT)改变及其影响因素。方法老年性白内障患者23例(27只眼),女性17例,男性6例,平均年龄(72±9.2)岁。行超声乳化白内障吸出人工晶状体植入手术。分别于术前及术后1 d、2周、1个月、3个月使用深度增强相干光断层扫描(EDI-OCT)测量SFCT及眼压(IOP)。比较不同性别之间SFCT变化的差异,使用回归分析了SFCT改变与年龄、眼轴(AL)及IOP的关系。结果术前SFCT平均厚度(201.2±45.8)μm,,术后1 d、2周、1个月和3个月SFCT值分别是(213.1±47.9)μm、(223.8±49.4)μm、(238.7±54.9)μm和(221.0±50.0)μm,术后各时间点SFCT值和术前比较差异均有统计学意义(P<0.01)。统计分析表明SFCT改变与性别、年龄、AL和IOP无显著相关。结论超声乳化白内障吸出术后3个月内SFCT增厚,其中1个月最明显。SFCT值的改变与性别、年龄、AL及IOP均无显著相关。

Objective To evaluate subfoveal choroidal thickness（ SFCT） after phacoemulsification,and to analyze factors related to its changes. Methods Twenty-seven eyes of 23 patients with senile cataract were enrolled in this prospective study. Subjects included 6 males and 17 females. Average age was 72. 0 ± 9. 2 years. SFCT and intraocular pressure（ IOP） were measured preoperatively,1 day,2 weeks,1 month and 3 months after surgery. SFCT measurements were performed with enhanced depth imaging optical coherence tomography（ EDI-OCT）. The difference in SFCT between genders was analyzed. Regression analysis was conducted to identify factors,including age,axial length（ AL） and IOP,that might result in the changes of subfoveal choroidal thickness. Results Mean SFCT before surgery was 201. 2 ± 45. 8μm. After surgery,mean SFCT was 213. 1 ± 47. 9μm,223. 8 ± 49. 4μm,238. 7 ± 54. 9μm and 221. 0 ± 50. 0μm at 1day,2 weeks,1 month and 3 months,respectively. Increase in SFCT after surgery was statistically significant（ P〈0. 01）. Statistical analysis revealed that changes of SFCT were not related with factors such as sex,age,IOP or AL. Conclusions Mean SFCT increased after phacoemulsification,and reached its peak at 1 month. The changes of SFCT are not related with sex,age,AL or IOP.
