摘要
目的探讨MRI在儿童神经管发育畸形筛查诊断中的应用价值。方法选取产前接受神经系统功能检查的546例儿童作为研究对象,分别行超声和MRI检查;比较不同检测方法下神经管畸形儿童判定结果、与手术结果符合率以及各自诊断信息。结果本组546例共检出59例儿童存在神经管发育畸形,且均经手术证实。其中二种方法检出结果一致52例,7例不符,以手术结果为标准,MRI检查符合率98.72%,诊断特异性为98.99%,敏感性为95.92%,漏诊率为4.08%,误诊率为1.01%。59例畸形儿主要分布在新生儿~12岁,其中50例经手术缓解,9例死亡。结论 MRI在诊断儿童神经管发育畸形中具有重要的意义,其能清晰显示脊柱软组织区及脊髓栓系各组织情况,为确保下一代健康,儿童在接受常规超声检查的同时再接受MRI检查,以便深入了解神经管畸形程度、评估脂肪瘤范围,为临床麻醉及手术提供更为可靠的信息。
Objective To investigate the value of MRI tube malformation screening in the diagnosis of children's nerve.Methods A total of 546 cases of children accepted prenatal nervous system function tests were selected as the research subjects,respectively,were examined by MRI and ultrasound;Comparison of different methods for detecting neural tube deformity children determination results,surgery and the coincidence rate and their diagnostic information.Results All of 546 cases were detected in 59 cases of children with neural tube malformations,and were confirmed by surgical operation.The two methods of detection results of 52 cases,7 cases were inconsistent with surgical results for the standard MRI examination,the coincidence rate was 98.72%,specificity was 98.99%,sensitivity was 95.92%,the misdiagnosis rate was4.08%,the misdiagnosis rate was 1.01%.There were 59 cases of malformation mainly in the newborn to12 years,including 50 cases after surgical remission,9 cases of death.Conclusions MRI in the diagnosis of children with neural tube malformation has important significance,it can clearly show the spinal soft tissue area and tethered spinal cord tissues,in order to ensure the health of the next generation,children in routine ultrasound examination at the same time to accept MRI.In order to understand neural tube deformity,lipoma assessment,for clinical anesthesia and surgery to provide more reliable information.
