摘要
目的:采用基于体素的形态学分析方法(voxel-based morphometry,VBM)探讨初发未治疗青少年癫痫患者脑灰质体积的变化特点。方法:对20例初发未用药青少年癫痫患者及20例年龄、性别及教育程度相匹配的健康对照组行高分辨磁共振T1W(3DT1W)检查,采集数据后,采用SPM8中的VBM8软件进行图像分析,统计分析采用两样本t检验比较青少年癫痫与健康对照的脑灰质体积差异。结果:与正常对照组相比较,初发未用药青少年癫痫患者组灰质体积增加的脑区包括:右侧海马及海马旁回、右侧岛叶、右侧扣带回、右侧中央前回和左侧海马旁回,没有发现灰质体积减少的区域。结论:初发未用药青少年癫痫已在多个脑区出现灰质体积的改变,这些脑区的改变可能与癫痫患者的临床症状及初始发病部位有关。
Objective: To investigate the gray matter volume change in the first-episode drug-naive adolescent epilepsy patients by using voxel-based morphometry analysis. Methods: Twenty cases first-episode drug-naive adolescent epilepsy patients and 20 cases of age-,sex-and education-matched healthy controls were included in our present study. All participants were performed high resolution magnetic resonance examination and all the 3DT1 W structural data were analyzed based on the VBM8 and SPM8 software. Two-sample t-test was used to observe the regional cerebral gray matter volume abnormalities between the adolescent epilepsy patients and the healthy controls. Results: Compared with normal control,adolescent epilepsy patients showed increased gray matter volume on the right hopocampus and parahippocampal gyrus,right insula,right cingulate gyrus,right precentral gyrus and the left parahippocampal gyrus.No decreased gray matter volume was found in our study. Conclusion: Multiple brain regions of increased gray matter volume were found in first-episode,drug-naive adolescent epilepsy patients which may be correlated with the clinical symptoms of patients and the epilepsy onset position.
