经阴道超声在诊断子宫内膜病变的应用价值

Clinical Value of Transvaginal Ultrasound in Diagnosis of Endometrial Lesions

为探讨经阴道超声诊断子宫内膜病变的临床价值,选取2013年6月至12月疑诊为子宫内膜病变的患者111例,所有患者均先行阴道超声检查,并进行诊断性刮宫术或行宫腔镜检查取出组织,送组织病理学检查。分析其超声声像图表现,并与病理结果进行对照,评价阴道超声检查的诊断符合率。结果为:111例患者中,组织病理学证实子宫内膜息肉48例,经阴道超声检查诊断子宫内膜息肉40例,诊断检出率分别为43.24%和36.03%;病理结果子宫内膜增生45例,经阴道超声检查诊断子宫内膜增生42例,诊断检出率分别为40.54%和37.84%;病理结果子宫内膜癌18例,经阴道超声检查诊断子宫内膜癌13例,诊断检出率分别为16.22%和11.71%。由检出结果可知,组织病理学检出率与阴道超声检出率无统计学差异(P>0.05)。经阴道超声检查因简单易行、无创伤、费用低廉而易于为患者所接受,且检出率接近于组织病理学结果,可作为诊断子宫内膜病变的有效筛查手段。

Objective：to discuss the clinical value of transvaginal ultrasound diagnosis of endometrial disease. Method：to choose 111 patients suspected with endometrial lesions from June to December,2013, all patients were first examined with transvaginal ultrasound, and treated with diagnostic curettage or hysteroscopy to extract tissue, and the tissues were sent for histopathology examination. Their ultrasound images were analyzed, and the results were compared with pathological results to evaluate diagnosis coincidence rate of transvaginal ultrasound examination. Result：among 101 patients,48 with Endometrial polyps confirmed by histopathology, 40 with endometrial polyps by transvaginal ultrasound diagnosis, the diagnosis rate was 43.24% and 36.03 % respectively. 45 cases of endometrial hyperplasia by pathological results ,42 cases by transvaginal ultrasound diagnosis of endometrial hyperplasia, the diagnosis rate was 40.54% and 37.84% respectively. 18 cases with endometrial carcinoma by pathological results, 13 cases by transvaginal ultrasound diagnosis, the diagnosis rate was 16.22% and 11.71% respectively. The experimental results can be concluded that there was no sig- nificant statistical difference in the detection rate of histopathology and vaginal ultrasound （ P 〉 0.05 ）. Conclusion ： Transvaginal ultrasound examination is simple, non - invasive, inexpensive and easy for the patient to accept and detection rate is close to pathological results. As a result, transvaginal ultrasound can be used as an effective screening tool for the diagnosis of endometrial lesions.
