摘要
目的研究人工全髋关节置换术后双下肢长度不等长对髋关节功能的影响。方法选取2013年5月到2015年5月我院行人工髋关节置换术患者150例,然后行骨盆平片检查,测量双下肢长度,根据双下肢长度差值(limbs length discrepancy,LLD)将患者分为对照组41例(LLD≤5 mm)、Ⅰ组80例(5 mm<LLD≤10 mm),Ⅱ组20例(10 mm<LLD≤20 mm),Ⅲ组9例(LLD>20 mm),应用Harris评分标准评价各组髋关节功能,并比较各组满意度。结果对照组和组Harris评分显著高于Ⅱ组和Ⅲ组(P<0.05),对照组和Ⅰ组Harris评分比较差异无统计学意义(P>0.05),Ⅰ组和Ⅱ组Harris评分显著高于Ⅲ组,比较差异具有统计学意义(P<0.05);对照组满意率显著高于Ⅰ组、Ⅱ组和Ⅲ组,Ⅰ组满意率显著高于Ⅱ组和Ⅲ组,Ⅱ组满意率显著高于Ⅲ组,比较差异具有统计学意义(P<0.05)。结论人工全髋关节置换术后双下肢长度不等超过10 mm对髋关节功能具有一定影响。
Objective To study the effect of leg length discrepancy after total hip replacement on hip joint function.Methods We selected 150 cases who underwent artificial hip joint replacement in our hospital from May 2013 to May 2015,then examined plain films and measured the length of the double lower limbs. The patients were divided into control group( LLD≤5 mm,41 caess),group Ⅰ( 5 mm〈 LLD ≤10 mm,80 cases),group Ⅱ( 10 mm LLD≤20 mm,20 cases),groupⅢ( LLD 20 mm,9 cases) according to the difference of the length of the shorter limbs( LLD),hip joint function was evaluated by Harris score,the degree of satisfaction was compared. Results The Harris score of the control group and the group I was significantly higher than that in the group Ⅱ and the group Ⅲ( P〈0.05). There was no statistical significance between control group and group I( P〉0.05). The Harris score of group Ⅰ and group Ⅱ was significantly higher than that of groupⅢ. The difference was statistically significant( P〈0.05). The satisfaction rate of the control group was significantly higher than that of group Ⅰ,group Ⅱ and group Ⅲ. The satisfaction rate of group Ⅰ was significantly higher than that of group Ⅱand group Ⅲ. The satisfaction rate of group Ⅱ was significantly higher than that of group Ⅲ. The difference was statistically significant( P〈0.05). Conclusion Leg length discrepancy more than 10 mm after artificial total hip replacement had a certain effect on the function of hip joint.
