妊娠晚期阴道菌群的临床特征及治疗干预研究

Clinical Characteristics and Treatment of Vaginal Flora in Late Pregnancy

目的:探讨妊娠晚期阴道菌群的临床特征及治疗方案。方法:选取我院99例妊娠晚期患者,产前行阴道常规检查,检查阳性者按有无自觉症状分为R1组(有症状,33例)和R2组(无症状,33例),检查阴性者入R3组(33例),产前根据需要给予乳酸杆菌制剂。记录3组乳酸杆菌制剂用量、住院时间、产中及产后出血量,并分析阴道菌群分布特点,同时对比3组产妇结局。结果:R1组乳酸杆菌制剂使用时间明显多于R2组(P<0.05);3组住院时间、产中出血量、产后出血量比较无显著差异(P>0.05);阴道菌群检查假丝酵母菌37例,滴虫3例,细菌性阴道病5例,衣原体淋菌13例,淋菌8例;R1组产妇早产、胎膜早破、剖宫产、产褥感染发生率低于R2组,但差异不显著(P>0.05)。结论:妊娠晚期阴道菌群以假丝酵母菌、衣原体较常见,对阴道检查阳性且有症状者给予乳酸杆菌制剂治疗有效,但对无症状症状者效果不显著,临床需要综合考虑用药。

Objective： To investigate the clinical characteristics and treatment of vaginal flora in late pregnancy.Methods： Select our hospital 99 cases of late pregnant patients,routine prenatal vaginal examination.Examination was positive in according to whether the symptoms were divided into group R1（symptomatic,33 cases） and R2 group（asymptomatic,33 cases）,into the examination was negative in R3 group（33 cases）,prenatal according to the need of Lactobacillus.The dosage,length of stay,the amount of postpartum hemorrhage and the distribution of vaginal flora in 3 groups were recorded and compared with the 3 groups.Results： R1 group of Lactobacillus preparation time was significantly more than that of R2 group（P〈0.05）; 3 group,the hospitalization time,produced in the amount of bleeding,postpartum hemorrhage compared no significant difference（P〈0.05）; vaginal flora check Candida in 37 cases,3 cases of Trichomonas,5 cases of bacterial vaginosis,clothing original body gonococcus in 13 cases,8 cases of gonococcal; R1 groups of maternal premature delivery,premature rupture of membranes,cesarean section production,puerperal infection and lower incidence of R2 group,but the difference was not significant（P〈0.05）.Conclusion： Late pregnancy vaginal flora to Candida albicans,Chlamydia is common.On vaginal examination positive symptoms and give Lactobacillus treatment effective,but there was no effect on the symptoms,clinical need comprehensive consideration of the medication.
