白内障患者围术期的感染预防对控制术后眼内感染发生的影响

Effect of Perioperative Infection Prevention on Postoperative Intraocular Infection in Patients with Cataract

目的:探讨白内障患者实行围手术期的感染预防对控制术后眼内感染发生的影响。方法:186例白内障患者按消毒方式随机分成A、B、C三组,各62例。A组:采用生理盐水结膜囊冲洗方法消毒。B组:A组基础上术前再次聚维酮碘消毒后手术。C组:B组基础上,术后聚维酮碘消毒后直接包扎伤口。结果:病原菌阳性率:消毒前,A组为53.23%,B组为58.06%,C组为56.45%;消毒后,A组为11.29%,B组为12.90%,C组为9.68%,三组消毒后均比消毒前明显下降(P<0.05),但三组患者之间比较无显著差异(P>0.05)。消毒后,三组患者不良反应发生率无显著差异(P>0.05),消毒后,A组眼部不适症状发生率明显低于B、C组(P<0.05)。结论:白内障患者实行围手术期感染预防能有效的控制眼部病菌增长,减少术后眼内感染发生率。

Objective： To investigate the perioperative cataract patients to prevent infection on the occurrence of postoperative intraocular infection control effect.Methods： 186 cases of cataract patients according to the disinfection methods were randomly divided into A,B,C three groups,62 cases in each.Group A： using saline irrigation of conjunctival sac disinfection method.Group B： group A based on the preoperative again povidone iodine disinfection after surgery.Group C： group B based on the postoperative of povidone iodine directly after wound dressing.Results： The positive rate of pathogenic bacteria： before disinfection,group A was 53.2%,group B was 58.06%,group C was 56.45%; after disinfection,group A was 11.29%,group B was 12.90%,group C is 9.68%,three groups after disinfection were than before disinfection decreased significantly（P〈0.05）,but between the three groups of patients with no significant difference（P〈0.05）.After disinfection,there was no significant difference in the incidence of adverse reactions between the three groups（P〈0.05）.After disinfection,the incidence of ocular discomfort in group A was significantly lower than that in group B and group C（P〈0.05）.Conclusion： Perioperative infection prevention in patients with cataract can effectively control the growth of ocular pathogens and reduce the incidence of postoperative intraocular infection.
