通心络胶囊联合辛伐他汀对早期糖尿病肾病胱抑素C的影响

目的通过观察血浆胱抑素C水平的变化，了解通心络胶囊联合辛伐他汀治疗早期糖尿病肾病（DN）的临床疗效。方法80例早期DN患者随机分为治疗组和对照组，每组各40例。两组均给予医学营养治疗指导，血压和血糖达标后，在常规治疗的基础上，治疗组给予通心络胶囊、辛伐他汀口服；对照组单用辛伐他汀口服。分别于治疗前和治疗后8周检测空腹血糖（FBG）、糖化血红蛋白（HbAlc）、尿白蛋白排泄率（UAER）、血肌酐（sCr）、血脂、C反应蛋白（CRP）、胱抑素C（CysC）。结果两组治疗前后FBG、HbAlc、BP无明显变化（P均〉0．05）。治疗8周后，治疗组及对照组的UAER、sCr、CRP、CysC、血脂较治疗前均改善（P均〈0．05），且治疗组UAER、CysC低于对照组（P〈0．05）。结论通心络胶囊联合辛伐他汀治疗早期DN能更有效地改善肾脏的微循环，发挥抗炎作用，能有效降低胱抑素C，尿白蛋白排泄率等指标，延缓肾功能损害的进展，效果优于单用辛伐他汀。

Effect of Tongxinluo capsule combined with simvastatin on early diabetic nephropathy＇s cystatin C.Objective To observe the changes of plasma level s of cystatin C, understand the Tongxinluo capsule combined with simvastatin in the treatment of early diabetic nephropathy （DN） patients. Methods 80 ca ses of early DN were randomly divided into treatment group and control group, 40 cases in each group. The two groups were given medical nutrition thera py, controlling blood pressure and blood glucose, in on the basis of routine treatment, the treatment group was given Tongxinluo capsule and simvastatin o rally; control group was treated with simvastatin respectively. The two group＇s change s of fasting blood glucose （FBG）, glycosylated hemoglobin （HbAlc） , urinary albumin excretion rate （UAER）, serum creatinine （sCr）, blood lipid, C reaction protein （CRP）, Cystatin C （Cys-C） were observed before and after 8 weeks treatment. The results of the two groups before and after treatment FBG, HbAlc, BP did not change significantly （P 〈 0.05）. After 8 weeks of treat ment, the treatment group and control group, UAER, sCr, CRP, Cys-C, blood lipid was better than before treatment （P 〈 0.05）. And the treatment group U AER, C Ys-C lower than control group （P 〈 0.05）. Conclusions Tongxinluo capsule and simvastatin treatment of early DN can more effectively improve re nal microcirculation, anti-inflammatory effect, can effectively reduce the serum cystatin C and urinary albumin excretion rate and other indicators, delay pr ogression of renal damage results and in simvastatin alone.
