补阳还五汤治疗脑梗死恢复期气虚血瘀证的Meta分析

Buyang Huanwu Decoction in the Treatment of Cerebral Infarction Reconvalescents with Qi-deficiency and Blood-stasis Syndrome:A Meta Analysis

目的系统评价补阳还五汤治疗脑梗死恢复期气虚血瘀证病人的临床疗效和安全性。方法收集补阳还五汤治疗脑梗死恢复期气虚血瘀证的随机对照试验研究文献进行Meta分析。结果共纳入22篇文献(共2 123例研究对象),其中15项试验对临床总有效率进行了评定,15项试验对神经功能缺损评分进行了评定,11项试验对病人日常生活活动能力进行了评定,5项试验对中医证候疗效进行了评定。15项试验临床总有效率的Meta分析结果提示补阳还五汤组疗效优于对照组,RR=1.20,95%CI[1.14,1.26],P<0.000 01;15项试验神经功能缺损评分的Meta分析结果提示,补阳还五汤组对神经功能缺损的改善优于对照组,SMD=-0.93,95%CI[-1.17,-0.69],P<0.000 01;11项试验日常生活活动能力的Meta分析结果提示,补阳还五汤组优于对照组(SMD=0.73,95%CI[0.56,0.90],P<0.000 01;5项试验中医证候疗效的Meta分析结果提示,补阳还五汤组疗效优于对照组,RR=1.23,95%CI[1.10,1.37],P=0.000 2。结论补阳还五汤治疗脑梗死恢复期气虚血瘀证病人有明显疗效,且安全性较高。

Objective To evaluate the clinical curative effect and safety of Buyang Huanwu decoction（BHD）for treatment of cerebral infarction reconvalescents with qi-deficiency and blood-stasis syndrome.Methods The randomized controlled clinical trials（RCTs）literatures of BHD for treatment of cerebral infarction reconvalescents with qi-deficiency and blood-stasis syndrome were collected for meta analysis.Results Twenty-two literatures（involving 2 123 patients）were included.Among them,fifteen RCTs evaluated the clinical total effective rate,fifteen RCTs evaluated the neural function defect score,eleven RCTs evaluated the activities of daily life and five RCTs evaluated the total effective rate of Chinese medical syndromes.The meta analysis results showed that compared with the control group,the test group was superior in elevating the clinical total effective rate,RR =1.20,95%CI[1.14,1.26],P0.000 01,ameliorating neural function defect,SMD=-0.93,95%CI[-1.17,-0.69],P〈0.000 01,improving activities of daily life,SMD=0.73,95%CI[0.56,0.90],P〈0.000 01,and elevating the total effective rate of Chinese medical syndromes,RR =1.23,95%CI[1.10,1.37],P =0.000 2.Conclusion BHD has obvious effect on cerebral infarction reconvalescents with qi-deficiency and blood-stasis syndrome,and haVE a higher security.
