局部枸橼酸抗凝在酒精中毒致挤压综合征患者连续性血液净化治疗中的应用及护理

Application of regional citrate anticoagulation in continuous blood purification for patients with crush syndrome caused by alcoholism and related nursing measures

目的探讨局部枸橼酸抗凝技术在酒精中毒致挤压综合征患者连续性血液净化(CBP)治疗中的应用效果及相关护理措施。方法 5例酒精中毒致挤压综合征患者接受局部枸橼酸抗凝CBP治疗及护理,观察患者转归及治疗前后血尿素氮(BUN)、血肌酐(Scr)、血清磷酸激酶同工酶(CPK)、尿肌红蛋白(Mb)、血K+和尿量的变化情况。结果 5例患者肾功能恢复正常,治愈出院。患者治疗后BUN、Scr、CPK、Mb、血K+水平较治疗前降低,尿量较治疗前增加,差异有统计学意义(P<0.05或P<0.01)。结论局部枸橼酸抗凝技术对改善酒精中毒致挤压综合征患者的治疗安全有效。

Objective To investigate the efficacy of regional citrate anticoagulation in contin- uous blood purification （CBP） for patients with crush syndrome caused by alcoholism and to discuss the related nursing measures. Methods A total of 5 patients with crush syndrome caused by alcohol- ism were given CBP and related nursing measures. The blood urea nitrogen （BUN） , serum creatinine （Scr） and creatine phosphate kinase （CPK）, uric myoglobin （Mb）, serum K＋ and urine volume were compared before and after treatment. Results Five patients with recovered renal function suc- cessfully discharged from the hospital. All patients had lower levels of BUN, Scr, CPK, Mb and K ~ and higher urine volume after treatment when compared 0.01 ）.
