功能磁共振成像在脑结核瘤诊断中的应用研究进展 被引量:4

摘要 脑结核瘤临床表现复杂,误诊率高。近年发展起来的功能磁共振成像技术如磁共振波谱(MRS)、弥散加权成像(DWI)和灌注加权成像(PWI),可分别从生理代谢、微环境水分子扩散情况及血流动力学方面反映正常与病变组织的特点。脑结核瘤在MRS上常表现为脂质峰升高,伴或不伴胆碱/肌酸、胆碱/N-乙酰天门冬氨酸值轻度增高且无其他阳性代谢物;在DWI上的信号较为复杂,但病变区域的表观弥散系数(ADC)值及相对ADC值常高于对侧大脑半球正常脑实质。在PWI上病灶区域与正常脑白质的相对脑血容值<1可用于同其他类型肿瘤的鉴别。
