宫颈环切术治疗宫颈上皮内瘤变疗效及其对患者妊娠结局的影响

Curative effect of Cervical intraepithelial neoplasia and its impact on pregnancy outcome in patients with cervical ring resection

目的探讨宫颈环切术治疗宫颈上皮内瘤变疗效及其对患者妊娠结局的影响。方法选取行宫颈环切术治疗宫颈上皮内瘤变后成功妊娠产妇41例作为观察组,随机抽取同期41例子宫无病变的健康妊娠产妇41例作为对照组,观察宫颈环切术的临床疗效,并比较两组产妇妊娠结局、分娩孕周、分娩方式、新生儿出生体重与Apgar评分变化情况。结果术后随访,39例患者恢复良好,宫颈环切术治愈率达95.12%。两组产妇早产、产后出血等妊娠结局及分娩孕周比较,差异无统计学意义(P>0.05)。观察组产妇阴道分娩24(58.54)例,剖宫产17(41.46)例,与对照组产妇阴道分娩22(53.66)例,剖宫产19(46.34)例比较,差异无统计学意义(P>0.05)。观察组新生儿出生体重及Apgar评分分别为(3.69±0.54)g与(9.53±0.41)分;与对照组新生出生体重(3.63±0.58)g及Apgar评分(9.45±0.39)分比较,差异无统计学意义(P>0.05)。结论宫颈环切术应用于宫颈上皮内瘤变产妇临床治疗,对产妇妊娠结局及分娩方式无不良影响。

Objective To study the cure efficacy of cervical neoplasia loop excision procedure and its effect on pregnancy outcome in patients undergoing cervical intraepithelial neoplasia. Methods 41 patients who underwent cervical intraepithelial neoplasia and had been cured through cervical neoplasia loop excision procedure and then got successful pregnancy were selected as treatment group. 41 healthy pregnancy women with no disease were selected as control group. Efficacy of cervical neoplasia loop resection, was observed.Pregnancy outcomes, gestational age, and mode of delivery, birth weight of newborn and Apgar score changes were compared between two groups. Results According to postoperative follow-up, there were 39 patients who were well recovered,the cure rate was 95.12%.There were no statistically significant difference on pregnancy outcome including premature birth rate and postpartum hemorrhage rate and gestational age between two groups.（P〈0.05）. In treatment group, there were 24（58.54） cases having maternal vaginal delivery, there were17（41.46） cases having cesarean section. In control group there were 22（53.66） cases having vaginal delivery, and there were 19（46.34）cases having cesarean section. The difference between two groups was not statistically significant（P〈0.05）. In the treatment group, birth weight of babies and Apgar scores were（3.69 ± 0.54） g and（9.53 ± 0.41） points seperately; In the control group, newborn birth weight were（3.63 ± 0.58） g and Apgar scores（9.45 ± 0.39） points. The difference between two groups was no statistically significant（P〈0.05）.Conclusion Cervical neoplasia loop excision procedure can be used to cure cervical intraepithelial neoplasia, and there was no adverse effects on pregnancy outcomes and maternal delivery mode.
