会阴侧切分娩助产技术对产妇产后盆底功能的短期影响分析 被引量:3

摘要 目的:探讨会阴侧切分娩助产技术对产妇产后盆底功能的短期影响。方法:本次研究选取于2013年1月至2015年7月间我院收治的经阴道自然分娩的足月产妇进行回顾性分析,共计316例。按照助产技术分为会阴侧切分娩助产(A组)137例,非会阴侧切(B组)179组。分析会阴侧切分娩助产技术对产妇产后盆底功能的短期影响。结果:发生侧切产妇盆底肌肉肌力损伤概率较大,侧切产妇(A组)为(114/137),对比非侧切产妇(57/179)差异具有显著性,P<0.01。同时,侧切产妇发生阴道疼痛概率较高(51/137),与非侧切产妇相比(19/179)差异具有显著性,P<0.01。会阴侧切分娩后压力性尿失禁概率大于非侧切,差异具有显著性,P<0.01;会阴侧切产妇发生盆腔脏器脱垂的比例高于非侧切产妇,差异具有显著性,P<0.01。比较侧切产妇与非侧切产妇发现,前者盆底肌肉疲劳程度较高,肌电值较小,且差异均具有显著性,P<0.01。结论:使用会阴侧切助产技术后对产妇盆底功能造成一定的损伤,临床使用过程中应限制该技术的使用,亦可在产前对产妇进行详尽的检查,避免技术滥用对产妇的健康和正常生活造成影响。 Objective:To investigate the short-term impact of episiotomy midwifery childbirth postpartum pelvic floor function. Methods.. This study selected in between January 2013 to July 2015 in our hospital natural vaginal delivery at term women were retrospectively analyzed a total of 316 cases. According to midwifery into episiotomy midwifery childbirth (A group) 137 cases of non-episiotomy (group B) 179 groups. Analysis of episiotomy birth midwifery short-term impact on postpartum pelvic floor function. Results: The occurrence of lateral maternal pelvic floor muscle strength damage probability of large, lateral maternal (A group) to 114/137, compared to non-lateral maternal (57/179) difference was significant,P 〈 0.01. Meanwhile, the lateral vaginal pain of maternal high probability (51/137), compared to non-lateral maternal (19/179) difference was significant, P 0.01. Perineum after childbirth lateral stress incontinence probability is greater than the non-lateral, the difference was signifi- cant, P 〈 0.01; pelvic organ prolapse higher than the proportion of non-lateral maternal, the difference was significant,P〈 0.01. Comparison of lateral and non-lateral maternal maternal found that the higher the former pelvic floor muscle fatigue, electromyography smaller value, and the difference was significant, P〈 0.01. Conclusion: The use of episiotomy midwifery maternal pelvic floor causing some damage, the clinical use of the process should be limited to the use of this technology, also in maternal prenatal make a thorough examination, to avoid abuse of the technology and maternal health affecting normal life.
