摘要
目的对冠状动脉侧支循环形成的影响因素进行Logistic回归分析,为治疗冠状动脉侧支循环形成患者提供科学的数据参考。方法选取浙江省金华市人民医院2012年12月~2015年12月间190例冠心病患者作为研究对象,依据冠状动脉造影检查结果分为侧支生成组(100例)和无侧支生成组(90例),采取Logistic回归方法分析患者的性别、年龄、高血压和糖尿病等指标。结果侧支生成组IL-8为(12.50±1.58)ng/mL,无侧支生成组IL-8为(3.30±0.49)ng/mL,差异有统计学意义(P〈0.05)。侧支生成组PF4为(5.8±1.2)ng/mL,无侧支生成组PF4为(12.5±1.9)ng/mL,差异有统计学意义(P〈0.05);侧支生成组与无侧支生成组均存在冠状动脉血管闭塞情况,侧支生成组完全闭塞率为87.0%,无侧支生成组完全闭塞率为25.6%,侧支生成组的完全闭塞率高于无侧支生成组,差异有统计学意义(P〈0.05);IL-8、PF4与冠状动脉侧支循环生成具有相关性。结论冠状动脉侧支循环主要出现在严重冠心病中,冠状动脉病变的严重程度和IL-8以及PF4均是造成侧支循环生成的重要因素。
Objective To analyze the influencing factors of formation of coronary collateral circulation by Logistic regression, and to provide scientific data for the treatment of patients with coronary collateral circulation. Methods Selected 190 cases of coronary heart disease patients as research subjects in the people's hospital of Jinhua city from December 2012 to December 2015, based on the results of coronary angiography, patients were divided into collateral formation group(100 cases) and no collateral formation group(90 cases), patient's gender, age, hypertension and diabetes were analyzed by Logistic regression method. Results The IL-8 in collateral formation group was(12.50±1.58) ng/m L,and IL-8 of no collateral formation group was(3.30 ±0.49) ng/m L, the difference was statistically significant(P〈0.05).PF4 of collateral formation group was(5.8±1.2)ng/m L, PF4 in no collateral formation group was(12.5±1.9) ng/m L, the difference was statistically significant(P〈0.05); there were coronary artery occlusion in collateral formation group and without collateral generation group, the complete obliteration rate was 87.0% in collateral formation group and no collateral to generate the set of complete occlusion rate was 25.6%, the complete occlusion rate of the collateral formation group was much higher than that of no collateral formation group, the difference was statistically significant( P〈0.05);The IL-8 and PF4 levels were associated with coronary collateral circulation showed a correlation. Conclusion Coronary collateral circulation mainly in severe coronary artery disease, the severity of coronary artery disease and IL-8,and PF4 are important factors causing formation of collateral circulation.
