玻璃体切除联合曲安奈德玻璃体注射治疗增殖期糖尿病视网膜病变的临床研究

Clinical study of vitreous body resection combined with triamcinolone acetonide vitreous body injection for the treatment of proliferative diabetic retinopathy

目的对玻璃体切除联合曲安奈德玻璃体注射治疗增殖期糖尿病视网膜病变的治疗效果进行评价。方法选择2013年1月~2015年1月在我院住院的治疗的增殖期糖尿病视网膜病变患者60例（66眼）作为研究对象,根据治疗方法不同分为联合组（n=30,玻璃体切除联合曲安奈德玻璃体注射治疗）和单纯手术组（n=30,玻璃体切除术）,比较两组的视力改善情况及前房炎症反应情况。结果观察组治疗后3例视力变化≤1行,视力改善率达60.0%;对照组治疗后9例视力变化≤1行,视力改善率40.0%,观察组的视力改善率显著高于对照组,差异有统计学意义（P〈0.05）。观察组治疗后前房炎症反应较轻,明显轻于对照组,差异有统计学意义（P〈0.05）。结论玻璃体切除联合曲安奈德玻璃体注射治疗增殖期糖尿病视网膜病变可以明显改善患者的视力,减轻炎症反应,值得推广和应用。

Objective To evaluate the effect of vitreous resection of the therapeutic triamcinolone acetonide combined with vitreous injection in treatment of proliferative diabetic retinopathy. Methods From January 2013 to January 2015 in our hospital, 60 cases of patients with（66 eyes） were as the research object, and were divided into combination group（n =30, vitreous surgery combined with injection of triamcinolone acetonide vitreous body） and surgery alone group（n =30, vitrectomy） according to different treatment methods. Visual improved and inflammatory reaction in anterior chamber were compared between two groups. Results After treatment, 3 cases of visual acuity was less than or equal to 1,visual improvement rate of 60.0%; 9 cases visual acuity was less than or equal to 1 in the control treatment group, visual improvement rate was 40.0%, the visual improvement rate in observation group was significantly higher than that of the control group, the difference was statistically significant（P〈0.05）. After treatment, the anterior chamber inflammation reaction was light, was significantly lighter than the control group, the difference was statistically significant（P〈0.05）.Conclusion Vitreous resection combined with triamcinolone acetonide vitreous injection in treatment of proliferative diabetic retinopathy lesions can significantly improve the patient＇s vision, reduce inflammation, worthy of promotion and application.
