以定标活检技术评价胃痞方合黄芪针穴注治疗慢性萎缩性胃炎脾胃虚弱证的疗效

Evaluation of Clinical Efficacy of Weipi Decoction Plus Astragalus Injection Acupoint Injection for Chronic Atrophic Gastritis with Spleen-Stomach Deficiency Type Based on Marking Targeting Biopsy

【目的】基于定标活检技术(MTB)评价胃痞方合黄芪注射液穴位注射治疗慢性萎缩性胃炎(CAG)脾胃虚弱证的疗效。【方法】将64例CAG脾胃虚弱证患者随机分为治疗组和对照组各32例。2组均给予抗幽门螺杆菌(Hp)感染等常规处理,治疗组同时给予胃痞汤合黄芪注射液穴位注射治疗,对照组同时给予胃复春治疗,12周为1个疗程。观察2组的主要症状改善情况及临床证候疗效,并在定标活检下观察胃镜下病理改变情况。【结果】(1)主要症状改善情况:治疗12周后,治疗组胃脘痞满、胃脘胀痛、胃脘嘈杂等主要症状的改善均优于对照组(P<0.05)。(2)临床证候疗效:治疗组总有效率为93.8%,优于对照组的53.1%,差异有统计学意义(P<0.05)。(3)内镜疗效:治疗组总有效率为93.8%,对照组为87.5%,2组内镜疗效比较,差异无统计学意义(P>0.05)。【结论】胃痞方合黄芪注射液双侧足三里穴位注射可显著改善慢性萎缩性胃炎脾胃虚弱证患者的临床症状。

Objective To evaluate the clinical efficacy of Weipi Decoction plus Astragalus Injection acupoint injection for chronic atrophic gastritis（CAG） with spleen-stomach deficiency type based on marking targeting biopsy（MTB）. Methods A total of 64 CAG patients with spleen-stomach deficiency type were randomized into treatment group and control group,32 patients in each group. Both groups were given routine treatment such as counteracting Helicobacter pylori（Hp）infection,and additionally, the control group was given oral use of Weifuchun Tablets, and the treatment group was given oral use of Weipi Decoction plus acupoint injection with Astragalus Injection on bilateral Zusanli（ST36）. Twelve weeks covered one treatment course. After treatment,the effects on the main clinical symptoms and clinical syndromes were evaluated,and the changes of gastric mucosa were observed under gastric endoscope by MTB. Results（1） After treatment for 12 weeks,the relief of gastric distention and fullness,gastric distending pain, gastric discomfort in the treatment group was superior to that of the control group（P〈0.05）.（2） The total effective rate for clinical syndromes in the treatment group was 93.8%, and was 53.1% in the control group, the difference being statistically significant（P〈0.05）.（3）The total effective rate for gastroendoscopy in the treatment group was 93.8%,and was 87.5% in the control group,the difference being not statistically significant（P〉0.05）. Conclusion Weipi Decoction plus Astragalus Injection acupoint injection is effective on obviously relieving the clinical symptoms of CAG with spleen-stomach deficiency type.
