降钙素原对指导重症手足口病使用抗生素的价值 被引量:2

The Value of Procalcitonin in Guiding the Use of Antibiotics during the Treatment of Severe HFMD

摘要 目的:分析重症手足口病伴有神经系统损伤患儿降钙素原检查结果,为早期识别危重症手足口病患儿提供指导。方法:回顾性分析2015年4-8月桂林医学院附属医院儿科92例伴有神经系统症状重症手足口病患儿的一般临床资料和降钙素原检查测定数据,选取PCT<0.5 ng/m L者58例为对照组,采用常规治疗;选取PCT≥0.5 ng/m L者34例为试验组,在常规治疗的基础上,加用抗生素治疗,疗程3 d左右,比较两组的临床疗效。结果:试验组与对照组相比,发热、惊跳、外周血白细胞数等症状明显好转,病程显著缩短,两组比较差异均有统计学意义(P<0.05)。结论:血清降钙素原对指导重症手足口病早期应用抗生素有重要的作用,明显提高了对重症手足口病早期的识别及处理,大大地增加了重症手足口病早期治愈率,降低了致残率及死亡率。 Objective:To analyze the result of serum procalcitonin(PCT) in children who had severe hand,foot and mouth disease(HFMD) with neurological damage,it can provide an early recognition of severe HFMD.Metrod:The general clinical datas and serum PCT value of 92 children who had server hand,foot and mouth disease(HFMD) complicated by neurological damage in the affiliated hospital of Guilin medical university were analyzed from April to August in 2015.58 cases with PCT0.5 ng/m L were selected as the control group,they were given conventional treatment;34 cases with PCT≥0.5 ng/m L were selected as the treatment group,they were given antibiotics on the basis of above conventional treatment,the treatment course was about 3 days.The clinical curative effect of two groups were compared.Result:Compared with the control group,the treatment group's condition of fever, startle and peripheral blood leukocyte count were better and the course was significantly shorter,the differences were statistically significant(P〈0.05).Conclusion:PCT plays an important role for guiding the early use of antibiotics to severe HFMD,significantly improves the early identification and treatment of severe HFMD,greatly increased the cure rate of early severe HFMD,and reduces morbidity and mortality.
