乙型肝炎病毒不同基因亚型对患者免疫功能的影响比较

摘要 目的比较乙型肝炎病毒的不同基因亚型对患者免疫功能的影响。方法选择慢性乙型肝炎患者377例(血清HBVDNA均为阳性),留取血清标本。用巢氏PCR法检测所有入选者的HBV基因型亚型,ELISA法检测HBV不同基因型亚型感染者血清中IL10及IL17含量,全自动生化仪检测所有入选者的肝功能指标。结果377例患者HBV基因型亚型分型结果:C型323例,其中C2亚型321例(85.1%),C1亚型2例(0.5%);B型41例(10.9%),均为B2亚型;BC混合型12例(3.2%);D型1例(0.3%)。该组B2亚型、C2亚型、BC混合型HBV感染者的性别、年龄、ALT、AST、TBIL指标差异均无统计学意义(均P〉0.05)。B2亚型、C2亚型、BC混合型HBV感染者的IL17水平差异无统计学意义(P〉0.05),感染HBVB2亚型、C2亚型者的IL10水平差异有统计学意义(F=0.444,P〈0.05)。结论HBV不同基因型亚型对机体免疫功能可能有不同的影响。 Objective To compare the influence of different genotypes of hepatitis B virus to the body'simmune function. Methods 377 patients with chronic hepatitis B virus ( serum HBV DNA are all positive) wereselected. The serum specimens were collected. Nest PCR method was used to detect all entrants subtype HBVgenotypes, ELISA method was used to detect serum levels of IL - 10 and IL - 17, fully automatic biochemicalinstrument was used to test all entrants liver function. Results Of 377 patients with chronic hepatitis B, C genotypein 323 cases, C2 type in 321 cases ( 85. 1%) , C1 type 2 cases ( 0. 5%) ; Genotype B 41 cases ( 10. 9%) , were B2subtype; B and C mixed 12 cases ( 3. 2%) ; Type D in 1 case ( 0. 3%) . The sex, age, ALT, AST, TBIL of B2, C2subtypes, BC mixed infection had no statistically significant differences. B2, C2 subtypes, BC mixed infection had nostatistically significant difference in serum IL - 17 level. And B2, C2 subtype infection had statistically significantdifference in IL -10 level ( F =0. 444, P 〈0. 05) . Conclusion Different genotype HBV subtypes may have differenteffect to the body's immune function.
