腹腔镜下卵巢囊肿剔除电凝止血与缝合止血对卵巢功能的影响研究

目的探讨腹腔镜下卵巢囊肿剔除电凝止血与缝合止血对卵巢功能的影响。方法选取2014年1~5月本院收治的110例行腹腔镜下卵巢囊肿剔除术患者作为研究对象,随机分为A组与B组,各55例。A组采用双极电凝止血,B组采用缝合止血,比较两组术前、术后1个月、术后3个月及术后半年的激素水平,同时比较两组术前术后的卵巢功能状况。结果两组术后1、3个月的E_2、FO水平显著低于术前,LH、FSH水平显著高于术前,差异有统计学意义（P〈0.05）。两组术后半年的E_2、LH、FSH、FO水平与术前比较,差异无统计学意义（P〉0.05）。A组术后1、3个月的E_2、FO水平显著低于B组,LH、FSH水平显著高于B组,差异有统计学意义（P〈0.05）。两组术后半年的卵巢功能状况比较,差异无统计学意义（P〉0.05）。结论在腹腔镜下卵巢囊肿剔除术中采用缝合止血,可减少对患者卵巢功能的影响,有利于术后卵巢功能恢复,值得临床推广应用。

Objective To explore the influence of electric coagulation hemostasis and suture hemostasis in laparoscopic ovarian cyst removal on ovarian function. Methods 110 patients underwent ovarian cyst removal under laparoscope from January 2014 to May in our hospital were selected and randomly divided into group A and group B,55 cases in each group.Group A was given electric coagulation hemostasis,group B was given suture hemostasis.The level of hormone at preoperative,1 month,3 months and 6 months after operation was compared,the ovarian function in the two groups before and after operation was compared. Results The level of E_2,FO at 1 month and 3 months after operation in the two groups was lower than that before operation,the level of LH,FSH at 1 month and 3 months after operation in the two groups was higher than that before operation,with significant difference（P〈0.05）.The level of E_2,FO at 1 month and 3 months after operation in group A was lower than that in group B,the level of LH,FSH at 1 month and 3 months after operation in group A was higher than that in group B,with significant difference（P〈0.05）.There was no significant difference in the condition of ovarian function at 6 months after operation between the two groups（P〈0.05）. Conclusion Using suture hemostasis in ovarian cyst removal under laparoscope can reduce the influence on patients′s ovarian function,which is helpful to the recovery of postoperative ovarian function,it is worthy of clinical promotion and application.
