棘突椎板回植联合椎弓根螺钉固定治疗椎管内肿瘤 被引量:6

摘要 目的探讨棘突椎板回植联合椎弓根螺钉固定治疗椎管内肿瘤临床疗效。方法采用棘突椎板回植联合椎弓根螺钉固定治疗14例椎管内肿瘤患者。结果 14例均获得随访,时间8~16个月。行X线或CT复查显示棘突椎板复合体回植后达到解剖复位,局部连续性骨痂形成。术后无骨不连、后凸畸形、再发性椎管狭窄等并发症。按JOA评分:优9例,良3例,可2例。结论棘突椎板回植联合椎弓根螺钉固定治疗椎管内肿瘤,稳定脊柱,符合脊柱的生物力学特点,减少术后因瘢痕增生粘连导致的并发症,疗效满意。 Objective To explore the clinical effect of spinal canal tumors treated with the replantation of lamina and spinous process combined with pedicle screw fixation technique. Methods 14 patients of spinal canal tumors were treated by replantation of lamina and spinous process combined with pedicle screw fixation technique. Results All 14 patients were followed up for 8 ~ 16 months. X-ray or CT examination showed that the lamina complex of the spinal process reached anatomical reduction,and the local continuity of callus was formed. There were no postoperative complications such as nonunion,deformity and recurrent spinal stenosis. According to JOA score: excellent in 9 cases,good in 3,fair in 2. Conclusions The treatment of spinal canal tumors with the replantation of lamina and spinous process combined with pedicle screw fixation technique,which is not only maintain the stability of the spine,at the same time consistent with the biomechanical characteristics of spine,reduce operation due to scar adhesion caused by complications,curative effect is satisfaction.
