健康青年言语诱发听性脑干反应与最大言语识别率的关系 被引量:1

Relationships between electrophysiological characteristic of speech evoked auditory brainstem response and mandarin monosyllable discriminative ability

摘要 目的探讨言语诱发听性脑干反应(speech evoked auditory brainstem response,s-ABR)与最大言语识别率(phonetically balanced maximum,PB_(max))之间的相互关系。方法测试健康青年41耳的PB_(max)及其对应强度的s-ABR,分析s-ABR主波潜伏期和基频(fundamental frequency,F_0)幅值、第一共振峰(first formant,F_1)幅值与PB_(max)的关系。结果以PB_(max1)=100%、100%<PB_(max2)<90%及PB_(max3)≤90%分为3个组,结果显示3组PB_(max)及其s-ABR F_0、F_1幅值和各主波潜伏期的差异有统计学意义(P<0.05)。对s-ABR的主波潜伏期和F_0、F_1幅值与PB_(max)进行相关分析,结果显示s-ABR电生理特性与PB_(max)均表现出显著相关性(P<0.05),各主波潜伏期与PB_(max)负相关,而F_0幅值和F_1幅值分别与PB_(max)正相关。结论s-ABR的电生理特性和言语识别功能关联紧密,这些电生理特性有望成为评价言语识别功能的客观指标,并可能联合或者单独应用于言语识别功能的相关研究。 OBJECTIVE To investigate the relationships between electrophysiological characteristic of speech evoked auditory brainstem response and Mandarin phonetically balanced maximum,so as to provide more clues for the mechanism of speech cognitive behavior.METHODS The speech discrimination scores were obtained by Mandarin phonemic-balanced monosyllable lists via speech audiometric software in forty-one ears of normal hearing adults.Their s-ABRs were recorded with speech syllables da with the intensity of phonetically balanced maximum(PBmax).The electrophysiological characteristic of s-ABR,as well as the relationships between PBmax and s-ABR parameters including latency in time domain,fundamental frequency(F_0) and first formant(F_1)in frequency domain were analyzed statistically.RESULTS While divided the subjects into three groups by PB_(max1)=100%,100%PB_(max2)90%and PB_(max3)≤90%,the results showed a significant differences of s-ABR parameters including latencies of all feature peaks,amplitudes of F_0and F_1(P0.05).Amplitudes of F_0 and F_1 showed a strongest significant positive correlation with PBmax respectively,and latencies of all the feature peaks showed a significant negative correlation with PBmax.CONCLUSION These electrophysiological characteristics of s-ABR showed a close association with Mandarin monosyllable discriminative abilities,which may be formed as an objective auditory measure to assess speech recognition,and may be applied jointly or separately for further investigation of speech perception and temporal processing abilities.
