冠心病患者PTCA术前后无症状与有症状心肌缺血的对比分析

Contrast between Symptomatic and Asymptomatic Myocardial Ischemia of Patients with Coronary Heart Disease before and after PTCA

目的：观察冠心病患者经皮腔内冠脉成形（ＰＴＣＡ）术前后有症状与无症状心肌缺血总负荷的变化。方法：用动态心电图心肌缺血总负荷对１２０例ＰＴＣＡ术治疗的患者进行术前术后对比分析。结果：ＰＴＣＡ术前无症状心肌缺血总负荷是有症状心肌缺血总负荷的４２倍；ＰＴＣＡ术后近期和远期有症状与无症状心肌缺血总负荷均较术前显著改善（Ｐ均＜００１），术后尚存的心肌缺血事件中９０６％为无症状心肌缺血。

Objective:To observe the total ischemic burden(TIB)of symptomatic and asymptomatic myocardial ischemia in patients with coronary heart disease before and after percutaneous transluminal coronary angioplasty(PTCA) Methods:Using the TIB measured by holter monitoring to analyse 120 patients before and after PTCA Results:The TIB of asymptomatic myocardial ischemia was four times as much as that of symptomatic myocardial ischemia before PTCA The TIB of both symptomatic and asymptomatic myocardial ischemia after PTCA were significantly reduced in comparison with that before PTCA(P<0.01).90.6% of cases with myocardial ischemia were silent(asymptomatic)after PTCA Conclusion:It is clinically significant to follow-up the cases with asymptomatic myocardal ischemia after PTCA
