76例急性心力衰竭中医证型与原发病及心功能等级关系系统综述

A Summary of 76 Cases of Acute Heart Failure with TCM Syndrome and Primary Disease and Heart Function

[背景]心力衰竭中医证型与原发病及心功能等级关系密切。[目的]分析76例急性心力衰竭中医证型、原发病及心功能等级。[资料来源]在湖北省孝感市中心医院心内科选取2013年5月至2015年6月间76例急性心力衰竭患者病历。[选择文献量及依据]1研究类型:临床观察。2研究对象:符合急性心力衰竭诊断标准。中医四诊资料齐全;同一患者反复入院症状发生变化时重复纳入,无明显变化时仅纳入第一次入院情况;知情同意,签署知情同意书。3评价指标:中医证型、原发病、心功能等级。4干预方法:入院后立即给予抢救,持续高流量吸氧,详细记录一般资料,性别、年龄、中医证型、原发病、心功能等级等,记录发病时症状表现及检查报告,根据临床症状对症治疗,同时镇静、扩冠等,抢救成功,当日或次日送入住院部。排除中医四诊资料不全;未遵医嘱要求出院。共76例住院病历。[数据提炼规则及应用方法]使用Excel2010软件建立数据库,将急性心力衰竭患者临床资料进行统计,分析中医证型、原发病、心功能等级。[数据综合得出结果与结论]气虚血瘀24例,气阴两亏17例,心肺气虚14例,阳虚水停11例,阴竭阳脱10例。急性心力衰竭常合并高血压、冠心病、糖尿病等。气虚血瘀型心功能Ⅱ级数量最高,气阴两亏、阳虚水停及阴竭阳脱心功能Ⅳ级数量最高。[未来展望]扩大样本量,深入研究,分析急性心衰患者中医证型及临床特点,对治疗干预具有重要指导意义。

[Background]The relationship between TCM syndrome types of heart failure and primary disease and heart function is closely related.[Objective]To analyze 76 cases of acute heart failure TCM syndrome,primary disease and heart function.[Data sources]In the Department of Cardiology of Xiaogan Central Hospital of Hubei province from May 2013 to June 2015,76 cases of acute heart failure patients.[Selection of literature quantity and basis]①Research type：clinical observation.②Research object： In accordance with the diagnostic criteria of acute heart failure.TCM four diagnostic information is complete;the same patients with recurrent symptoms during admission changed repeatedly into the,no significant changes included only the first admission;informed consent,signed the informed consent.③Evaluation index：TCM syndrome type,primary disease,cardiac function class.④Intervention method：After admission to the rescue,sustained high flow oxygen,detailed records of general information,sex,age,Chinese medicine syndrome type,the original onset,heart function,etc.,record the symptoms and inspection reports,according to clinical symptoms and symptomatic treatment,while sedation,expansion,etc.,to save success,the day or the next day into the hospital.Exclusion of TCM diagnostic data insufficiency;noncompliance discharge requirements.A total of 76 cases of hospitalized medical records.[Data extraction rules and application method]Using Excel2010 software to establish a database,the clinical data of acute heart failure patients were statistically analyzed,the TCM syndrome type,the original disease,heart function grade.[Results and conclusion of the data]24 cases of Qixu Xueyu,17 cases of Qiyin Liangkui,14 cases of Xinfei Qixu,11 cases of Yangxu Shuiting,10 cases of Yinjie Yangtuo.Acute heart failure often combined with hypertension,coronary heart disease,diabetes,etc.Qi deficiency and blood stasis type heart function is the highest,Qi Yin deficiency,Yang deficiency and water retention and the function of the two levels of the Yin and Yang deficiency.[Future Outlook]Expand the sample size,in-depth study,analysis of TCM Syndrome Types and clinical characteristics of acute heart failure patients,the treatment of intervention has important guiding significance.
