医护一体化诊疗模式对个案追踪考核效果的影响

Effect of diagnosis and treatment mode of integrated medicine and nursing on assessment results of case tracking

目的通过医护一体化的工作模式提高护士个案追踪考核效果。方法选择危重疾病患者60例,根据实施医护一体化诊疗模式对危重患者全方位的跟踪、分析,较真实地发现危重患者全过程中的护理问题,并进行持续质量改进。结果实施个案追踪考核后,护理管理质量(包括病房管理、分级护理、基础护理、护理安全、健康教育)及护理文书合格率情况均显著提高(P<0.05)。结论在病区中持续开展医护诊疗合作模式能提高护理人员的个案追踪考核能力,提高护理质量。

Objective To improve the assessment effect of the case tracking by the diagnosis and treatment mode of integrated medicine and nursing. Methods A total of 60 critically ill patients were selected and conducted with diagnosis and treatment mode of integrated medicine and nursing.Nursing problems of critically ill patients were observed,tracked and analyzed,and continuous quality improvement was performed. Results After implementation of the case tracking,the qualified rates of nursing management（ such as ward management,grade nursing,basic nursing,nursing safety and health education） and nurse care records significantly improved（ P〈0. 05）. Conclusion Implementation of the diagnosis and treatment mode of integrated medicine and nursing can improve the ability of case tracking of the nurses,and enhance the quality of nursing.
