三棱针刺络放血拔罐联合理疗治疗腰椎间盘突出随机平行对照研究

The Randomized Parallel Contrast Study of Blood-Letting Cupping Combined Physical Therapy in Treating Lumbar Disc Herniation

[目的]观察三棱针刺络放血拔罐联合理疗治疗腰椎间盘突出疗效。[方法]使用随机平行对照方法,将240例门诊患者按按就诊顺序号方法简单随机分两组。对照组120例腰部两侧肌肉物理治疗,电流强度大小因各体差异不,因人而异局部有酸麻,舒适为主,1次/d,40min/次。治疗组120例三棱针刺络放血拔罐,选腰阳关(浅刺)、气海俞(中刺)、大肠俞(中刺)、腰俞(浅刺)、命门(浅刺)、悬枢(浅刺)、腰眼(浅刺)、秩边(深刺)、环跳(深刺)、小肠俞(中刺)、阿是穴,常规消毒,避免感染,对准穴位直刺,然后用拔罐法,造成负压,可让瘀血排出更充分,1次/周;物理治疗同对照组。连续治疗21d为1疗程。观测临床症状、不良反应。治疗1疗程,判定疗效。[结果]治疗组治愈36例,有效58例,无效26例,总有效率78.33%。对照组治愈21例,有效51例,无效48例,总有效率60.00%。治疗组疗效优于对照组(P<0.05)。[结论]三棱针刺络放血拔罐联合理疗治疗腰椎间盘突出,疗效满意,无严重不良反应,值得推广。

[Objective] Observing the clinical effect of blood-letting cupping combined physical therapy in treating Lumbar Disc Herniation. [Method]We used the randomized parallel contrast method and 240 clinic patients with thoracic outlet syndrome were randomly divided into 2 groups according.The 120 patients in control group used waist muscle physical therapy, 40 minutes at a time,once a day.The current strength size varies from person to person,always feeling limp and numb.The 120 patients in treatment group used blood-letting cupping blood-letting therapy once a week,choosing yaoyangguan （shallow- needing）, qihai shu （middle needing）, dachangshu （middle needing）, yaoshu （shallow needing）, mingmen （shallow needing）, xuanshu （shallow needing）, yaoyan（shallow needing） ,zhibian（deep needing）, huantiao （deep needing）, xiaochangshu （middle needing）,ashi point.First we did regular disinfection to avoid infection and used acupuncture point stab, then by cupping method to cause the negative pressure which can let the blood stasis to discharge more fully.In addition, physical therapy is the same as the control group,21 days for a course of treatment.During this time we observed clinical symptoms and adverse reactions to judge curative effect.[Result]Treatment group cured 36 cases, 58 cases effectively, 26 cases had no effect, the total effective rate was 78.33%.Control group cured 21 cases, 51 cases effectively and 48 cases had no effect, total effective rate was 60.00%.The treatment group curative effect is better than the control group（P〈O.O5）.[Conclusion]Bloodletting cupping combined physical therapy to treat Lumbar Disc Herniation, obtained satisfactory therapeutic results and no serious adverse reactions, which is worth to be popularized.Key words： bloodletting and cupping, physiotherapy, Lumbar Disc Herniation
