手术和非手术治疗重症急性胰腺炎临床对比观察

The effect of surgical and non-surgical treatment of severe acute pancreatitis

目的:对比观察重症急性胰腺炎手术和非手术治疗的临床效果。方法:选取重症急性胰腺炎患者78例,随机分为对照组和观察组分别39例,其中对照组患者手术治疗,观察组患者非手术治疗,对比两组患者的临床治疗效果。结果:对照组治愈率为69.2%,死亡率为30.8%,观察组治愈率为87.2%,死亡率为12.8%,对照组治愈率、死亡率和观察组比较,差异具有统计学意义(P<0.05);手术组20例患者发病48 h内进行手术治疗,死亡率为15.0%,48 h后进行手术治疗的患者中死亡率为47.4%,48 h内手术患者病死率明显低于发病48 h后的患者,差异具有统计学意义(P<0.05)。观察组并发症发生率明显低于对照组,差异具有统计学意义(P<0.05)。结论:重症急性胰腺炎采用非手术治疗其病死率较低,同时对于手术治疗的患者尽快实施手术能够降低患者的死亡率。

Objective to explore the effect of surgical and non-surgical treatment for severe acute pancreatitis.Methodchoose 78 patients with severe acute pancreatitis were randomly divided into control group and observation group 39 cases respectively,the control group patients surgery,observation group of patients with non-surgical treatment,compared two groups of patients with the clinical therapeutic effect.Results the control group cure rate was 69.2 %,the mortality was 30.8 %,the observation group cure rate was 87.2 %,the mortality rate of 12.8%,compared to control group cure rate,mortality,and observation group,difference has statistical significance（P〈0.05）;Scores within 48 h of the 20 patients underwent surgery treatment,the mortality rate of 15.0 %,after 48 h of surgical treatment of patients with a mortality rate of 47.4 %,the rate of surgery patients within 48 h of lower than patients after 48 h of stroke onset,signifi cant difference statistically signifi cant（P〈0.05）.The incidence of postoperative complications of observation group was obviously lower than the control group,signifi cant difference has statistical signifi cance（P〈0.05）.Conclusion severe acute pancreatitis with non-surgical treatment the case fatality rate is low,at the same time as soon as possible for surgical treatment of patients can reduce the mortality of patients with surgery.
