血清Hepcidin水平在老年缺铁性贫血和慢性病贫血诊断中的临床应用 被引量:7

Application of Serum Hepcidin in Diagnosis of Elderly Iron Deficiency Anemia and Anemia of Chronic Disease

摘要 目的:探讨血清铁调素(Hepcidin)在老年人群缺铁性贫血(IDA)和慢性病贫血(ACD)中的应用价值。方法:选择老年IDA患者32例,老年ACD患者34例,健康对照组28例。检测对比三组血清Hepcidin、血清铁(SI)、血清铁蛋白(SF)、C-反应蛋白(CRP)、红细胞数(RBC)、血红蛋白(Hb)、红细胞比容(HCT)、红细胞平均体积(MCV)、白细胞数(WBC)水平。结果:ACD组中WBC明显高于IDA组(P<0.05)。血清Hepcidin在IDA组、ACD组及对照组之间具有显著性差异(P<0.05),IDA组中Hepcidin含量比对照组低,而ACD组中Hepcidin含量比对照组高(P<0.05)。IDA组、ACD组中SI明显低于对照组(P<0.05),而两组间无显著性差异(P>0.05)。ACD组中SF、CRP均明显高于IDA组与对照组(P<0.05)。Hepcidin诊断IDA组的ROC曲线下面积(AUC)值大于诊断ACD组,IDA组的诊断指数高于ACD组。结论:血清Hepcidin含量的检测对于鉴别IDA和ACD具有重要的意义。 Objective: To explore the application value of serum hepcidin in elderly iron deficiency anemia (IDA) and anemia of chronic disease (ACD). Methods: A total of 32 elderly IDA patients, 34 elderly ACD patients, and 28 healthy people ( as control group) were selected. The levels of serum hepeidin, serum iron (SI), serum ferritin (SF), e-reactive protein (CRP), red blood eount(RBC), hemoglobin(Hb), hematocrit(HCT), mean corpuscular volume(MCV) and white blood count(WBC) in the three groups were detected and compared. Results: The level of WBC in ACD group was significantly higher than that of IDA group (P〈0.05). There was significant difference in the levels of serum hepcidin in IDA,ACD and control group (P〈0.05),and the level of serum hepcidin in IDA group was lower than that of the control group, while the ACD group was higher than control group (P〈0.05). The levels of SI in ACD and IDA groups were obviously lower than that in control group (P〈0.05), but there was no significant difference between ACD group and IDA group (P〉0.05). The levels of SF and CRP in ACD group were obviously higher than those in control group and IDA group(P〈0.05). Hepcidin diagnosis: the area under curve (AUC) value of ROC of IDA group was higher than that of ACD group,and the diagnosis index of IDA was higher than that of ACD. Conclusion: The detection of serum hepcidin content has important significance for the identification of IDA and ACD.
