宫腔镜电切术联合孕激素治疗早期子宫内膜癌对患者生育功能的影响 被引量:22

摘要 目的探讨宫腔镜电切术联合口服孕激素及宫内放置左炔诺孕酮宫内缓释系统(LNG-IUS)治疗早期子宫内膜癌的效果及可行性。方法2010年9月1日至2012年8月30日间西北妇女儿童医院妇科病例22例根据时间分组,均实施宫腔镜电切术,术后孕激素组单纯口服甲地孕酮片,LNG-IUS组单纯宫内放置LNG-IUS,联合组同时口服用药和宫内置药,观察3组患者缓解、复发、妊娠、不良反应等情况。结果3组患者完全缓解、无效、复发、妊娠和不良反应比较差异无统计学意义(χ2=1.027,P均〉0.05)。孕激素组、LNG-IUS组和联合组缓解时间分别为(10.8±2.1)、(10.5±1.8)、(9.5±1.8)个月,差异有统计学意义(F=6.944,P〈0.01),两两比较联合组缓解时间均短于另两组(t值分别为2.679,2.190,P均〈0.05);妊娠时间分别为(19.8±6.3)、(18.4±5.8)、(15.2±5.3),差异有统计学意义(F=5.143,P〈0.01),两两比较联合组显著短于另两组(t=3.194,P〈0.01,t=2.300,P〈0.05)。结论宫腔镜电切术联合口服孕激素及宫内放置LNG-IUS对于早期子宫内膜癌治疗具有有效性、可行性和安全性,但应注意治疗适应证。
